摘要
目的:探讨重度颅脑损伤治疗中丙氨酰-谷氨酰胺的应用价值。方法:选取2015年1月-2017年2月本院接诊的64例重度颅脑损伤患者为研究对象,利用电脑随机双盲法,分成试验组和对照组,各32例。其中试验组应用丙氨酰-谷氨酰胺[0.5 g/(kg·d)]强化治疗方案,对照组应用常规营养治疗方案。观察两组营养代谢指标的变化情况,比较D-乳酸水平和相关免疫指标。结果:试验组治疗后D-乳酸水平为(5.13±2.76)mg/L,明显低于对照组的(8.56±3.37)mg/L,组间差异有统计学意义(P<0.05)。试验组营养代谢指标和免疫指标的改善情况均优于对照组,组间差异有统计学意义(P<0.05)。结论 :对重度颅脑损伤患者应用丙氨酰-谷氨酰胺治疗,可显著改善其营养状况,增强机体免疫力,提高蛋白质合成量,值得临床借鉴。
Objective:To explore the application value of alanyl-glutamine treated with severe craniocerebral injury. Methods:64 cases of patients with severe craniocerebral injury were chosen as research objects from January 2015 to February 2017 in our hospital. They were divided into experimental group(n=32) and control group(n=32) according to computer random double blind method. The experimental group was administrated intensive treatment regimen of alanyl-glutamine [0.5 g/(kg·d)], and the control group was administrated the conventional nutrition treatment regimen. The changes of nutrient metabolism index of two groups were observed, and the levels of D-lactate and related immune indexes were compared.Results:The D-lactic acid level of experimental group was(5.13±2.76) mg/L, significantly lower than those in the control group(8.56±3.37) mg/L, and the difference between groups was statistically significant(P<0.05). The improvement of nutritional index and immunological index in the experimental group was better than those in the control group, and the difference between groups was statistically significant(P<0.05). Conclusion:The treatment of severe craniocerebral injury by alanyl-glutamine can significantly improve the nutritional status, enhance body immunity and improve protein synthesis amount, which is worth of clinical reference.
