分析肿瘤化疗患者的感染调查以及相关的危险要素

摘要 目的探究影响肿瘤化疗患者的感染危险因素,针对危险因素制定相应的措施,以减少感染发生。方法选取我院2015年5月至2018年11月间收治的肿瘤化疗患者424例为研究对象,采用回顾性分析总结患者临床资料和感染情况,统计患者感染部位以及感染原因,采用SPSS18.0进行统计学分析以及多因素logistic的回归分析,对患者感染情况制定相关的应对措施。结果根据调查显示,424例患者中出现87例患者出现感染情况,感染率为20.51%,感染部位多集中在呼吸道感染。依据多因素logistic的回归分析,年龄、住院天数和合并糖尿病、侵入性操作都是肿瘤化疗患者感染的独立危险因素,且呈现正相关关系(P<0.05)。主要病原菌为革兰阴性菌,检出病原菌112株,其中28株革兰阳性菌、78株革兰阴性菌、6株真菌,分别占25.00%、69.64%、5.35%。结论医院肿瘤化疗患者感染多数由于医院环境以及治疗过程中的操作引起,多数都是非人为因素,可进行人为干预进行改善,加强对危险因素的管理,保证患者最大的生命健康,针对危险因素做好预防措施。 Objective To explore infection risk factors of cancer patients with chemotherapy and formulate corresponding measures based on risk factors to reduce infection incidence.Methods Choose 424 cases cancer patients with chemotherapy admitted to our hospital from May 2015 to November 2018 as research objects.Review,analyze and summarize clinical data and infection situation of patients.Carry on statistical analysis on infection sites and causes,and multivariate logistic regression analysis with SPSS 18.0,formulat related measures based on infection situation of patients.Results According to investigation,87 of 424 cases had infection,with infection rate of 20.51%.Most of infected sites were respiratory tract infection.According to multivariate logistic regression analysis,age,length of stay,complicated diabetes mellitus and invasive operation were independent infection risk factors of cancer chemotherapy patients,with positive correlation(P<0.05).Main pathogenic bacteria include gram-negative bacteria.112 strains of pathogenic bacteria were detected,of which 28 were Gram-positive bacteria,78 were Gram-negative bacteria and 6 were fungi,accounting for 25.00%,69.64%and 5.35%respectively.Conclusion Most of infections of chemotherapy cancer patients in hospital were caused by hospital environment and treatment operation instead of human factors.Human intervention can be carried out to strengthen risk factors management,ensure maximum life and health of patients,and take preventive measures against risk factors.
