摘要
目的研究妊娠期糖尿病导致巨大儿发生的相关因素。方法选择我院2013年1月至2019年2月期间收治的妊娠期糖尿病产妇2662例作为研究对象。根据母婴结局分为巨大儿组(283例)、非巨大儿组(2379例)。对比分析两组基础治疗、孕期体质量情况、血糖情况。结果两组患者在巨大儿分娩史、新生儿男婴占比、诊断时体质量、分娩前体质量、孕期增重以及OGTT 1h的对比具有显著性差异(P<0.01);巨大儿组接受治疗率明显低于非巨大儿组(P<0.05);接受治疗的患者当中,巨大儿组采用饮食治疗联合胰岛素治疗的占比明显低于费巨大儿组(P<0.05。结论妊娠期糖尿病孕妇与发生巨大儿之间的联系因素主要包含巨大儿分娩史、新生儿男婴占比、分娩前体质量、孕期增重以及OGTT1h,临床必须要高度重视妊娠期糖尿病患者的临床管理工作,通过科学合理的血糖管理、体重管理,告知患者临床治疗的重要性,给予饮食治疗的同时,还需要给予胰岛素治疗,从而有效减少巨大儿的产生。
Objective To study the related factors of gestational diabetes and macrosomia.Methods A total of 2662 women with gestational diabetes who were admitted to our hospital from January 2013 to February 2019 were selected as subjects.According to the maternal and child outcomes,they were divided into a large group(283 cases)and a non-macro group(2379 cases).Comparative analysis of the two groups of basic treatment,body weight during pregnancy,blood sugar.Results There were significant differences between the two groups in the history of childbirth,the proportion of newborn male infants,the quality of the body at the time of diagnosis,the quality of the pre-delivery,the weight gain during pregnancy,and the OGTT 1 h(P<0.01).The treatment rate was significantly lower than that of the non-macro group(P<0.05).Among the patients treated,the proportion of the diet group treated with diet therapy was significantly lower than that of the fetus group(P<0.05).Conclusion Gestational diabetes The relationship between pregnant women and the occurrence of giant children mainly includes the history of childbirth,the proportion of newborn males,the quality of delivery,the weight gain during pregnancy and the OGTT1 h.The clinical management of patients with gestational diabetes must be highly valued.Scientific and rational blood sugar management,weight management,to inform patients of the importance of clinical treatment,while giving diet treatment,also need to give insulin treatment,thus effectively reducing the production of giant children.
