后路椎间孔镜下开窗减压髓核摘除术治疗单节段神经根型颈椎病 被引量:2

Fenestration and Decompression Nucleus Pulposus Enucleation in Treatment of Single-segment Cervical Spondylotic Radiculopathy under Posterior Foramen Endoscopy

摘要 目的探讨在单节段神经根型颈椎病病人治疗中采取后路椎间孔镜开窗减压髓核摘除术的治疗效果。方法对照组病人采取颈椎前路行椎间盘切除联合内固定术治疗,观察组病人采取后路椎间孔镜椎间盘髓核摘除术治疗。结果观察组病人手术耗时、术中平均出血量和住院时间等手术指标均较对照组病人更优(P<0.05);观察组病人术后颈椎曲度和椎间高度的恢复效果均较对照组病人更优(P<0.05);观察组病人术后1-6月的VAS评分均较对照组病人更低(P<0.05)。结论在单节段CSR病人治疗中,采取后入路椎间孔镜下行开窗减压髓核摘除术有利于改善病人颈椎病变并可降低术后疼痛度,该手术方案值得在临床中应用并推广。 Objective To investigate curative effect of fenestration and decompression nucleus pulposus enucleation in treatment of patients with single-segment cervical spondylotic radiculopathy under posterior foramen endoscopy. Methods Control group was treated with anterior cervical discectomy combined with internal fixation, and observation group with posterior transabdominal lumbar discectomy under foramen endoscopy. Results Operation time, average intraoperative bleeding and hospital stay of observation group was better than control group(P<0.05);recovery effect of postoperative cervical curvature and intervertebral height of observation group was better than control group(P<0.05);VAS scores of observation group were lower than control group 1-6 months after operation,(P< 0.05). Conclusion During treatment of single segment CSR patients, fenestration and decompression nucleus pulposus under posterior foramen endoscopy can improve cervical spondylosis and reduce postoperative pain degree, which is worthy of clinical application and promotion.
