摘要
目的探讨小儿下呼吸道感染中的抗生素合理应用。方法选择2016年6月至2017年6月我院收治的130例小儿下呼吸道感染患者作为研究对象,采集呼吸道分泌物进行细菌鉴定和药敏分析,针对革兰氏阳性菌、革兰氏阴性菌,以及药物对于肺炎链球菌和金黄色葡萄球菌、大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌、副流感嗜血杆菌等的敏感度进行分析。结果在对130例小儿吓唬呼吸道感染患儿中,118例中检出病原菌。其中革兰阴性菌78例(66.1%),革兰阳性菌40例(33.9%);亚胺培南对肺炎链球菌和金黄色葡萄球菌的敏感度均为100.0%,其次为左氧氟沙星和头孢作物,亚胺培南的敏感性最低;亚胺培南对大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌、副流感嗜血杆菌等的敏感度为100%,其次为哌拉西林/他唑巴坦和哌拉西林/他唑巴坦,头孢噻圬的敏感度最低。结论小儿下呼吸道感染中革兰阴性菌的比例较高,临床治疗过程中应根据细菌鉴定结果和药敏试验结果,合理选用抗菌谱窄的抗生素,减少广谱抗生素的使用,避免药物滥用。
Objective to investigate reasonable application of antibiotics for children with lower respiratory tract infection. Methods choose 130 cases lower respiratory tract infection children treated in our hospital from June 2016 to June 2017 as research objects, collect respiratory tract secretions for bacterial identification and drug susceptibility test, analyze sensitivity of medicine for gram positive and negative bacteria, streptococcus pneumoniae and staphylococcus aureus, escherichia coli, klebsiella pneumoniae, pseudomonas aeruginosa bacteria and hemophilus parainfluenzae. Results among 130 cases lower respiratory tract infected children, 118 cases were detected with pathogenic bacteria, including gram negative bacteria 78 cases(66.1%), gram positive bacteria 40 cases(33.9%); sensitivity of imipenem against streptococcus pneumoniae and staphylococcus aureus was 100%, followed by levofloxacin and cephalosporin crops, imipenem has the lowest sensitivity; sensitivity of imipenem against escherichia coli, klebsiella pneumoniae, pseudomonas aeruginosa and haemophilus parainfluenzae was 100%, followed by piperacillin/tazobactam, and cefotaxime has the lowest sensitivity. Conclusion for lower respiratory tract infection of children, gram negative bacteria has high proportion, during clinical course, it is necessary to apply reasonable narrow spectrum antibiotics based on bacteria identification and drug sensitivity test results, reduce application of broad-spectrum antibiotics, and avoid drug abuse.
