摘要
目的探讨化疗期乳腺癌改良根治术患者自我护理能力的影响因素及自护健康教育的干预效果。方法选取2014年7月至2017年7月间陕西省宝鸡市岐山县医院收治的1 100例行乳腺癌改良根治术患者为研究对象。根据数表法随机将患者分为研究组与对照组,研究组552例,对照组448例。研究组患者在常规护理干预基础上采用自护健康教育,对照组患者采用常规护理干预,护理干预时间3个月。分析化疗期乳腺癌患者自我护理能力的影响因素,比较两组患者干预前后的自我护理能力评分、汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分和生活质量评分。结果文化程度、负性情绪、卡式功能状态(Karnofsky)评分和家庭关系是化疗期乳腺癌改良根治术患者自我护理能力的影响因素,差异均有统计学意义(均P <0. 05)。干预前,两组患者自我护理能力评分、HAMA评分、HAMD评分和生活质量评分比较,差异无统计学意义(P> 0. 05)。干预后,研究组患者自我概念、健康知识水平、自我护理技能、自我护理责任感和自我护理能力总分均明显高于对照组患者,HAMA评分和HAMD评分均明显低于对照组患者,生理维度、情感维度、社会/家庭维度、功能维度评分及生活质量总分均明显高于对照组,差异均有统计学意义(均P <0. 05)。结论文化程度、负性情绪、Karnofsky评分和家庭关系均可能影响乳腺癌改良根治术患者的自护能力,而自护健康教育干预有利于提高患者的自我护理能力,改善负性情绪,提高生活质量,临床价值显著,值得推广应用。
Objective To explore the factors influencing self-care ability and intervention efficacy of self-care health education in patients undergoing modified radical mastectomy during chemotherapy.Methods A total of 1 100 patients undergoing modified radical mastectomy at Qishan County Hospital from July 2014 to July 2017 were enrolled in the study. According to the random number,patients were divided into a study group( 552 patients) and a control group( 448 patients). The study group received self-care health education besides routine nursing intervention and the control group received routine nursing intervention. The duration of nursing intervention was 3 months. Factors influencing self-care ability were analyzed.Self-care ability scores,Hamilton anxiety scale( HAMA) scores,Hamilton depression scale( HAMD)scores,and quality of life scores were compared between the two groups before and after the intervention.Results Education level,negative emotion,Karnofsky score and family relationship were the factors influencing self-care ability in patients undergoing modified radical mastectomy during chemotherapy. Before the intervention,there was no significant difference in self-care ability score,HAMA score,HAMD score,and scores quality of life( all P > 0. 05). After the nursing,self-concept,the level of health knowledge,selfcare skills,self-care responsibility,and the overall self-care ability scores were significantly higher in the study group than in the control group( all P < 0. 05). HAMA score and HAMD score were significantly lower in the study group than the control group( all P < 0. 05). Physiological,emotional,social/family and function scores and the overall score of quality of life were significantly higher in the study group than in the control group( all P < 0. 05). Conclusion Educational level,negative emotions,Karnofsky score,and family relationship may have an effect on the self-care ability in patients undergoing modified radical mastectomy.
