机械辅助自适应牵伸康复装置对脑卒中后前臂肌群功能的影响 被引量:2

摘要 目的观察机械辅助自适应牵伸装置对脑卒中后前臂旋前肌群功能的影响,探讨其改善前臂旋前功能障碍、促进手功能恢复的作用。方法选取脑卒中后上肢功能障碍患者120例,依据随机数表法分为对照组(A组)、机械辅助被动牵伸组(B组)、机械辅助自适应牵伸组(C组),均给予常规康复训练。在此基础上,B组给予机械辅助被动牵伸装置治疗,C组给予机械辅助自适应牵伸装置治疗。于治疗前(M0)、治疗1个月(M1)、治疗3个月(M3)后分别采用改良Ashworth痉挛量表(MAS)、上肢Fugl-Meyer运动功能评定量表(FMA)及改良Barthel指数(MBI)评分对三组患者进行疗效评定。结果在M0、M1时三组各项评分差异均无统计学意义(P>0.05)。在M3时C组在MAS、FMA评分与B组比较差异有统计学意义(P<0.05),而MBI评分差异无统计学意义(P>0.05);C组三项评分与A组差异有统计学意义(P<0.05);B组三项评分与A组均差异有统计学意义(P<0.05)。结论机械辅助自适应牵伸装置能够有效改善脑卒中后前臂旋前功能障碍,提高上肢运动功能。 Objective To observe the influence of mechanically assisted adaptive control drafting on forearm pronator spasticity and to explore its improvement on forearm pronator functional dysfunction and recovery of hand function.Me thods 120 post-stroke patients with dysfunction of upper limb were randomly divided into three groups,group A(the control group),B(mechanically assisted passive drafting group)and C(mechanically assisted adaptive control drafting group).All the patients in three groups were given conventional rehabilitation therapy.Patients in group B were trained by the device of mechanically assisted passive drafting while the ones in the group C were trained by mechanically assisted adaptive control drafting.The modified Ashworth scale(MAS),Fugl-Meyer assessment of upper limb(FMA)and modified Barthel index(MBI)were used to assess the curative effect at their admission at the hospital(M0),one month after admission(M1)and three months after admission(M3).Re s ults There was no significant difference in the three groups at three scores(P>0.05)at M0 and M1.At M3,the scores of MAS,FMA in group C were significantly better than those of group B(P<0.05)while the scores of MBI were not different between the two groups(P>0.05).There was significant difference between group C and group A(P<0.05)and between group B and group A at three scores(P<0.05).Conclus ion The device of mechanically assisted adaptive control drafting could effectively improve functional dysfunction of forearm pronator after stroke and improve the motor function of upper limbs.
