摘要 目的脑卒中后患侧前臂旋转功能障碍,严重影响了患臂和手的功能恢复。本文通过观察机械辅助牵伸对前臂旋前肌群痉挛的影响,探讨改善前臂旋转功能障碍对手功能恢复的作用。方法 73例脑卒中患者患侧上肢Brunnstrom 2期以上、前臂改良Ashworth痉挛评定≥1+,随机分为两组,两组都接受常规康复治疗,A组的前臂旋转牵伸由机械辅助进行。在入院时(M_0)、1个月后(M_1)及3个月(M_3)后评定肌张力、运动功能及日常生活活动能力。结果 M_0、M_1时两组上述三项评分比较差异无统计学意义(P>0.05),M_3时肌张力、运动功能评分A组明显好于B组(P<0.05),日常生活活动能力评分两组差异有统计学意义(P<0.05)。结论高强度的机械辅助牵伸能有效地控制前臂旋前肌群的痉挛,减轻了前臂的旋转障碍,促进了手功能的恢复,进而提高了患者的日常生活活动能力。 Objective Forearm rotation dysfunction of post-stroke patients seriously influenced the function recovery of dysfunction hand and arm.This study was to investigate the influence of mechanically assisted drafting on forearm pronator spasiticy and to explore its improvement on the recovery of hand function.Methods 73 post-stroke patients with hemiplegia,Brunnstrom stage of upper limbs 2 and 3,modified Ashworth assessment score of forearms ≥1 were randomly divided into two groups.Both groups were given conventional rehabilitation therapy.The forearm rotatation in group A was done with the help of mechanically assisted drafting.Muscle tone,motor function and activities of daily living were assessed at admission at the hospital(M0),one month after admission(M1) and three months after admission(M3).Results There was no significant difference between two groups at three scores( P〈0. 05) at M0 an M1.The muscle tone,motor function scores of group A was significantly better than that of group B( P〈0. 05) at M3.The activities of daily living showed significant statistical differences between two groups( P 0.005).Conclusion Mechanically assisted drafting effectively controlled forearm pronator spasticity and relieved the rotation dysfunction of forearm,facilitating the recovery of hand function and improving the activities of daily living of post-stroke patients.
