摘要
目的探讨成人血脂异常的认知、控制情况与供能营养素的关系,为血脂异常防治提供依据。方法通过多阶段分层抽样对北京等8个城市成人1658名进行横断面调查。采用多元Logistic回归对能量、三大营养素摄入量及其供能比与血脂异常的认知和控制情况进行相关性分析。结果健康组、无认知血脂异常组、控制较差组和控制良好组组间脂肪供能比(33.7%、32.0%、31.60%和35.4%)和碳水化合物供能比(54.9%、56.9%、56.85%和53.0%)相比较,差异有统计学意义(P<0.05)。多元Logistic回归显示,调整混杂因素后,碳水化合物供能比较低与无认知血脂异常组(OR=0.38, 95%CI:0.20~0.71)和控制较差组(OR=0.34, 95%CI:0.16~0.71)发病呈负相关,脂肪供能较低与无认知血脂异常组(OR=2. 28, 95%CI:1.19~4. 36)和控制较差组(OR=2.26, 95%CI:1.06~4.82)发病呈正相关。结论建议合理调整膳食结构尤其是脂肪和碳水化合物供能比,改善成年人对血脂异常的控制。
Objective To explore the association between awareness and control of dyslipidemia and energy supply nutrients,and provide evidence for comprehensive prevention and control of dyslipidemia.Methods A multi-stage stratified sampling cross-sectional survey had been conducted among adults in 8 cities of China with 1658 participates included.A multivariate logistic regression was carried out to analyze the association between energy,macronutrients and their energy supply ratio,also the awareness and control of dyslipidemia.Results Among 4 groups(health group,unaware group,poorly-controlled group and well-controlled group),the results of energy ratio of fat(33.7%,32.0%,31.60%,35.4%)and energy ratio of carbohydrate(54.9%,56.9%,56.85%,53.0%)were significantly different(P<0.05).Multivariate logistic regression showed that low carbohydrate energy supply ratio was negatively associated with unaware group(OR=0.38,95%CI:0.20-0.71)and poorly-controlled group(OR=0.34,95%CI:0.16-0.71),low fat energy supply ratio was positively associated with unaware group(OR=2.28,95%CI:1.19-4.36)and poorly-controlled group(OR=2.26,95%CI:1.06-4.82)adjusted for confounders.Conclusion This study proposes reasonable adjustments for dietary structure,especially energy supply ratios of fats and carbohydrates,in order to improve the control of dyslipidemia in Chinese adults.
