摘要
目的探讨基线血尿酸与臂踝动脉脉搏波传导速度(baPWV)纵向变化的关联。方法自2006-2007年起,由开滦总医院及下属医院共计11家医院,第1次对开滦集团公司在职及离退休职工进行健康体检。此后分别于2008-2009、2010-2011、2012-2013、2014-2015年进行第2、3、4、5次健康体检。自2010年起,在第3、4、5次体检时对参加体检的部分职工进行baPWV检测。对12 381例参加了开滦研究队列健康体检并完成两次baPWV检测的观察对象进行性别分层的线性混合效应模型分析,来观察基线血尿酸对baPWV纵向变化的影响。然后按血尿酸三分位对人群进行分组,继续采用线性混合效应模型分析不同水平血尿酸对baPWV纵向变化的影响。结果将12 381例观察对象按血尿酸三分位分组,3组人群的年龄、性别、随访时间、第1次与第2次baPWV、2次baPWV差值等指标的差异有统计学意义[血尿酸第1三分位组(低分位)比第2三分位组(中分位)比第3三分位(高分位)组:第1次baPWV,(1430.08±300.73)比(1482.74±310.93)比(1529.75±307.71)cm/s,第2次baPWV,(1481.81±331.97)比(1535.46±336.66)比(1566.34±320.85)cm/s,2次baPWV差值,(51.73±229.42)比(52.72±228.34)比(36.60±226.82)cm/s,均P<0.01]。性别分层的线性混合效应模型分析中,校正了年龄、随访时间等各种混杂因素后,发现男性中血尿酸与baPWV的纵向变化呈独立正相关(β=2.23,P=0.028),而女性中无明显相关;与血尿酸低分位组和血尿酸低、中分位组相比,仅男性血尿酸高分位组(血尿酸≥363μmol/L)与baPWV纵向变化呈独立正相关(β=6.07,P=0.012和β=5.81,P=0.007)。结论男性高水平基线血尿酸与baPWV的纵向变化呈正相关,且独立于血压、心率、肾小球滤过率、腰围等指标。
Objective To investigate the longitudinal association between baseline serum uric acid(SUA)and brachial ankle pulse wave velocity(baPWV).Methods From 2006 to 2007,the on-the-job and retired employees of kailuan group company participated health examination for the first time in 11 hospitals including Kailuan hospital and its affiliated hospitals.The second,third,fourth and fifth health examinations were conducted from 2008 to2009,2010 to 2011,2012 to 2013 and 2014 to 2015,respectively.Besides,since 2010,baPWV was examined in some employees who took part in the third,fourth and fifth health examination.Finally,12 381 individuals who participated in the health examination of Kailuan research cohort and completed two baPWV tests were analyzed by gender-stratified linear mixed-effects models to observe the effect of baseline SUA on the longitudinal changes of baPWV.Then,the subjects were grouped by SUA tertile,and the linear mixed effect model was used to analyze the effect of different levels of SUA on the longitudinal change of baPWV.Results There was a significant differences in age,gender,follow-up time,the first and the second baPWV,differences between twice baPWV and other factors among 3 groups(the first baPWV,(1430.08±300.73)vs(1482.74±310.93)vs(1529.75±307.71)cm/s,the second baPWV,(1481.81±331.97)vs(1535.46±336.66)vs(1566.34±320.85)cm/s,and differences between twice baPWV,(51.73±229.42)vs(52.72±228.34)vs(36.60±226.82)cm/s among 3 groups from low to high SUA tertile,all P<0.05)In gender-stratified linear mixed-effects models,there was an independent positive longitudinal correlation between SUA and baPWV in men(β=2.23,P=0.028)but not in women after adjustting for age,follow-up time and other confounding factors.Compared with the low or low/middle levels of SUA,the high levels of SUA(≥363μmol/L)were independently and positively correlated with the longitudinal changes of baPWV(β=6.07,P=0.012 andβ=5.81,P=0.007)in men.Conclusion Higher SUA was positively associated with baPWV in men but not in wom
