云南地区NK/T细胞淋巴瘤中Ig/TCR基因重排、EB病毒及免疫表型的检测

Detection of Ig/TCR gene rearrangements, immunophenotype and Epstein-Barr virus infection in NK/T-cell lymphoma in Yunnan

摘要 目的对云南地区NK/T细胞淋巴瘤的Ig/TCR基因重排、免疫表型以及EB病毒感染情况进行检测,为鉴别诊断NK/T细胞淋巴瘤提供依据。方法收集云南地区50例NK/T细胞淋巴瘤患者的肿瘤组织标本,免疫组化法检测LCA、CD79α、CD20、CD56、CD3、CD45RO、CD8、TIA-1、CD5抗体的表达;原位杂交法检测EB病毒编码的小分子RNA(EBER1/EBER2);BIOMED-2引物系统PCR扩增检测Ig/TCR基因的重排情况。结果 50例NK/T细胞淋巴瘤LCA、CD3、CD45RO、TIA-1、CD56、CD8和CD5阳性率分别为100%、76%、96%、98%、84%、28%和82%,CD79α和CD20均(-);原位杂交EBER阳性率为84%;基因重排检测Ig均为(-),TCR单克隆重排阳性率为16%。结论云南地区NK/T细胞淋巴瘤中存在少部分T细胞的单克隆性增生(16%);NK/T细胞淋巴瘤中大部分病例CD56为(+);EBER在鼻型NK/T细胞淋巴瘤的肿瘤细胞中呈高表达,提示可能为NK细胞来源。部分TCR基因单克隆重排阳性病例应为鼻NK样T细胞淋巴瘤。通过组织形态学、基因重排、免疫表型、EB病毒检测联合可以为NK/T细胞淋巴瘤的诊断提供可靠的依据。 Objective To detect the Ig/TCR gene rearrangements,the immunophenotype and Epstein-Barr virus(EBV)infection of NK/T cell lymphoma in Yunnan,to provide a basis for differential diagnosis of NK/T cell lymphoma.Methods The paraffin-embedded tissues of 50 patients with NK/TCL in Yunnan were collected and labeled with LCA,CD79α,CD20,CD56,CD3,CD45RO,CD8,CD5 and TIA-1 by immunohistochemical EnVision method to study their immunophenotype.In situ hybridization with EBER1/2 RNA probes was perfomed.Ig/TCR gene rearrangement was analyzed by polymerase chain reaction(PCR)technique using BIOMED-2 primer system.Results All the cases of NK/TCL were LCA positive.76%,96%,98%,84%,28%and 84%of the cases were positive for CD3,CD45RO,TIA-1,CD56,CD8 and CD5,respectively.EBER was dedected in 82%of the cases.No clonal Ig gene monoclonal rearrangements were detected in all the cases,but monoclonal TCR gene rearrangement was found in 16%(8/50)cases of NK/TCL.Conclusion There is TCR gene monoclonal T-cell proliferation in NK/TCL;CD56 is positive in most tumors.The high frequency of EBER infection in tumor cells of NK/T-cell lymphoma suggests that it may be a source of NK cells.The presence of clonal TCR gene rearrangement supports the diagnosis of nasal NK-like T-cell lymphoma.The histomorphologic and gene rearrangement analyses,immunophenotyping and EBV detection can provide a reliable basis for the diagnosis of NK/T cell lymphoma.
