摘要
目的探讨强直性脊柱炎(AS)患者辅助型T细胞1(Th1)、Th17和调节性T细胞(Treg)含量和相关细胞因子的表达变化及意义。方法选取2016年5月—2017年5月济宁医学院附属医院收治的30例AS患者为观察组,另选取同期30名健康人群为对照组。两组均检测Th1细胞、Th17细胞及Treg细胞在外周血淋巴细胞中的百分率,外周血中白介素1β(IL-1β)、IL-2、IL-6、IL-10、IL-17、IL-23及干扰素-γ(IFN-γ)、转化生长因子-β(TGF-β)指标水平;分析上述指标与AS活动度指数(ASDAS)的相关情况及指标之间相关性。结果观察组Th17细胞百分率和血清中IL-1β、IL-6、IL-17、IL-23等水平高于对照组(P<0.05),Th1细胞与Treg细胞百分率低于对照组(P<0.05)。IL-17、IL-23与ASDAS呈显著正相关(r=0.60、0.63,P<0.01),IL-10、TGF-β与ASDAS呈显著负相关(r=-0.37、-0.39,P<0.05);IL-17与IL-23呈显著正相关(r=0.91,P<0.01)。结论 AS患者外周血淋巴细胞Th17、Treg和Th1百分率变化导致T细胞亚群比例失调,从而导致发病或病情进展,并且与AS发病机制、活动性密切相关。
Objective To investigate the changes and value of Th1, Th17, Treg cells and related cytokines in patients with ankylosing spondylitis(AS). Methods Totally 30 AS patients in the Affiliated Hospital of Jining Medical Universitys from May 2016 to May 2017 were collected as the observation group, and 30 health subjedts were collected as the control group. The percentages of Treg cells, Th17 cells, and Treg cells in peripheral blood lymphocytes of the two groups were measured,and the levels of interleukin-1β, 2, 6, 10, 17, 23, IFN-γ, and TGF-β in peripheral blood were analyzed. Correlation with ASSDAS related conditions and indicators. Results The percentage of Th17 cells and serum levels of IL-1β, IL-6, IL-17, IL-23 were significantly higher in the observation group than those in the control group(P<0.05); the percentages of Th1 cells and Treg cells were significantly lower than those in the control group(P<0.05). There were significant positive correlations between IL-17, IL-23 and ASDAS(r=0.60, r=0.63, P<0.01). There were significant negative correlations between IL-10 and TGF-β and ADSAS(r=-0.37, r=-0.39.(P<0.05). IL-17 was positively correlated with IL-23(r=0.91, P<0.01). Conclusion The percentages of Th17,Treg and Th1 lymphocytes in peripheral blood of AS patients leads to the imbalance of T cell subsets, which leads to the onset or progression of the disease, and are closely related to the pathogenesis and activity of AS.
