核苷类似物抗病毒治疗在慢性重型乙型肝炎中的生存分析探讨

Investigating the Survival Analysis of Nucleoside Analogues Antiviral Therapy in the Treatment of Chronic Severe Hepatitis

摘要 探讨核苷类似物(NUCs)治疗慢性乙型重型肝炎(CSHB)的远期疗效以及影响CSHB远期生存率的相关因素。选择2001年1月至2009年12月住院的976例CSHB患者,均给予内科综合治疗,并依据治疗方法的不同分为加用NUCs治疗组(NUCs治疗组)和未加用NUCs治疗组(对照组),应用倾向指数卡钳匹配法对两组CSHB患者进行匹配,得到两组间各协变量均衡的样本,并用新样本做生存分析。经倾向性匹配后,共262对匹配成功,两组基线各指标差异无统计学意义(P>0.05)。NUCs治疗组的远期生存率及生存时间均大于对照组,差异有统计学意义(P<0.05)。多因素分析表明:性别、年龄、MELD值、胆碱酯酶、白细胞、肝性脑病、感染和NUCs应用与否均是影响患者生存率的独立预后因素,其中NUCs抗病毒治疗是一个时间依赖性的独立预后因素。NUCs治疗CSHB可以改善患者的远期预后,降低病死率。 To investigate the correlative factors and the long-term follow-up result about the survival analysis of nucleoside analogues( NUCs) antiviral therapy in the treatment of chronic severe hepatitis( CSHB),976 cases of CSHB patients during January2001 to December 2009 were selected. All received internal medicines comprehensive treatment. According to the different treating methods they were divided into adding NUCs treating group and non-NUCs treating group. The tendency index calipers matching method was used to match the CSHB patients of the two groups,and then the two groups' equilibrium sample of each covariate was obtained,and the survival analysis made by using the new sample. After the propensity matching,262 pairs were successfully matched,the two groups' baseline all indicated that they had no statistically significant difference( P > 0. 05). NUCs treatment groups' long-term survival rate and survival time were all better than those of the control group and the differences were statistically significant( P < 0. 05). Multiple factors analysis: Sex,age,MELD value,cholinesterase,white blood cell,hepatic encephalopathy,infection and the application of NUCs was the independent prognostic factors which infected the patient's survival rate,and the NUCs antiviral therapy was a time-dependent independent prognostic factor. Treating CSHB with NUCs can improve the long-term prognosis of the patients and reduce the cases' fatality rate.
