摘要
目的探讨强直性脊柱炎(ankylosing spondylitis, AS)髋关节病变的MRI表现及受累相关因素分析。方法回顾性分析95例AS患者临床及MRI资料,分析AS髋关节病变的MRI表现,并依据其特征,将本组病例分为髋关节受累组与未受累组,比较两组间临床及实验室检查中的差异。结果 95例患者中,MRI显示髋关节异常72例(75.8%)。44例(46.3%)髋关节滑膜炎,表现为滑膜增厚和(或)关节积液;19例(20.0%)关节面下骨质侵蚀;23例(24.2%)关节间隙异常;髋关节骨质增生、骨髓水肿及脂肪沉积分别为25例(26.3%)、31例(32.6%)、4例(4.2%);47例(49.5%)可见髋关节及其周围附着点炎,累及部位依次为耻骨联合、坐骨结节及股骨大粗隆;未见髋关节强直。统计分析表明,两组间发病年龄、髋关节症状阳性、CPR及ESR值差异具有统计学意义。结论 AS髋关节病变MRI表现有一定特征性,附着点炎及滑膜炎是最常见的表现。发病年龄、髋关节症状阳性、CPR及ESR水平与MRI髋关节受累相关。
Objective To investigate MRI manifestations of hip lesions of ankylosing spondylitis and related factors for hip involvement. Methods Clinical and MRI data of 95 AS patients were retrospectively reviewed, and MRI manifestations of AS hip lesions were analyzed. According to MRI features, patients were divided into hip involved group and non-involved group, differences between the two groups in clinical and laboratory examinations were compared. Results In 95 patients, to be MRI showed abnormal hip in 72 cases(75.8%). 44 cases(46.3%) showed hip synovitis, expressed as synovial thickening and(or) effusion;hip erosion was found in 19 cases(20%);23 cases(24.2%) showed abnormal hip joint space;hip hyperplasia, bone marrow edema and fat deposition were 25 cases(26.3%), 31 cases(32.6%), 4 cases(4.2%) respectively;47 cases(49.5%) showed hip and surrounding enthesitis, involved parts were pubic symphysis, sciatic tubercle, trochanter of femur;there was no hip ankylosis. Statistical analysis showed that age of onset, positive symptoms of the hip, CPR and ESR value were statistically significant between the two groups. Conclusion MRI features of AS hip lesions are characteristic, enthesitis and synovitis are the most common manifestations. Age of onset, positive hip symptoms, CPR and ESR levels were associated with MRI hip involvement.
