摘要
目的掌握经皮纤维环穿刺法和纤维环切开法建立兔椎间盘退变的模型,比较两种方法的特点,为后期建立邻椎病动物模型奠定实验基础。方法取10月龄健康新西兰大白兔20只,雌雄各10只,随机分为2组,A组为经皮纤维环穿刺组,即透视下经皮穿刺损伤目标间盘引起退变;B组为纤维环切开组,即切开皮肤暴露椎间盘,用手术刀片损伤间盘引起退变。所有实验动物术后在标准条件下饲养,观察其术后的精神情况、进食量、活动量、以及对外界刺激反应的情况并记录;每4周进行MRI检查,观察其退变情况。结果 A组手术前、后各时间点动物反应评分相比差异均无统计学意义(F=1.504,P>0.05);B组手术后第1、2和第3天的动物反应评分低于术前(F=6.313,P<0.01);术后第1、2和第3天的动物反应评分均A组低于B组(P均<0.01)。两组术后第6天的精神状况、进食量、活动量以及对外界刺激反应的评分均已接近术前(P>0.05)。造模成功后,动态查MRI,发现A、B两组实验动物从第4周开始,目标间盘在T2像上整体表现为信号下降,术后各时间点评分均低于术前(P均<0.01),随术后时间的延长,椎间盘高度有下降趋势,椎间盘退变表现越明显。结论两种方法均能引起兔椎间盘退变,简单且容易掌握;经皮纤维环穿刺法较纤维环切开法所致的术后反应更轻。
Objective To compare two methods for establishing Rabbit IVDD model through percutaneous annular fibrosus centesis and annulus incision respectively and prepare for further animal ASD model. Methods20 ten- months- old New Zealand healthy rabbits including ten males and ten females were randomly divided into two groups( named group A and group B). A group accepted the method of percutaneous annular fibrosus centesis under x- ray fluoroscopy,which destroyed the inter- vertebral disc leading to its degeneration. B group adopted annular fibrosus incision to ruin the disc for the disc degeneration using the scalpel. All experimental animals were feed by standard conditions post operation. Psychological status,food intake,activity levels and reply for surrounding stimulate were recorded postoperatively. MRI was used to observe the degree of inter- vertebral disc degeneration( IVDD) per 4 weeks. Results In group A,difference of pre- operation and post- operation reply scores at various points in time were not statistically significant( F = 1. 504,P >0. 05). However,baseline had significantly lower responses scores than it had at the 1st,2nd,3rdday after operation in group B( F = 6. 313,P < 0. 01). Moreover,results of response scores in group A were significantly less than those in group B at the 1th,2thand 3thday postoperative separately( t = 8. 196,5. 800 and 4. 016,all P < 0. 01). Performances of psychological state,food intake,activity levels and reply for surrounding stimulate in pre- operation were the same as those in post- operation while being in 6thday( P > 0. 05). After establishing rabbit IVDD model successfully,we found that it was four weeks for the target disc showing signal decline on MRI T2- weighted images in A and B group,the difference had statistically significant( F = 4. 157 or4. 512,P < 0. 05),and the changes were clear over time accompanied by reduction of height of the disc.Conclusion Two methods surely cause inter- vertebral disc degeneration of rabbit spine simply and easily.The percutaneous an
