解剖型锁定钢板结合微创技术治疗肱骨外科颈不稳定性骨折 被引量:4

摘要 目的探讨分析肱骨近端解剖型锁定钢板结合微创技术治疗肱骨外科颈不稳定性骨折的临床疗效。方法采用肱骨近端解剖型锁定钢板结合微创技术治疗肱骨外科颈不稳定性骨折45例,按Neer分型:II型骨折11例,III型骨折25例,IV型骨折9例。结果所有患者术中均未发生头静脉、腋神经及桡神经损伤,术后均未出现伤口感染、内固定松动或断裂、骨折不愈合以及肱骨头坏死等并发症,骨折临床愈合时间为4~12周,平均8周。按照Neer肩关节功能百分评分标准进行评定,优24例,良17例,中4例,差0例,优良率达91.1%。结论通过肱骨近端解剖型锁定钢板结合微创技术方法治疗肱骨外科不稳定性骨折,具有术中损伤小、出血少、骨折固定可靠、骨折愈合快、上肢功能恢复快、并发症少等优势,是治疗肱骨外科颈不稳定性骨折的理想方法。 Objective To analyze the clinical efficacy of anatomical locking plate combined minimally invasive techniques to treat the collum chirurgicum humeri unstable fracture.Methods A retrospective study for 45 cases of collum chirurgicum humeri unstable fracture which were treated by anatomical locking plate combined minimally invasive surgery. According to the Neer classification, there is 11 cases in II fracture, 25 cases in III fractures, 9 cases in IV fractures. Results Under surgery, all patients don't have cephalic vein, axillary nerve or radial nerve injury. After surgery, all patients don't meet other complications such as wound infection, loosening, facture, nonunion, humeral head necrosis and so on. All fractures were healed and union within 8 weeks average(range from 4 to 12 weeks). Evaluated by the standard of Neer shoulder score, the results showed 24 excellent, 17 good and 4 fair. Excellence rate reach to 91.1%(41/45). Conclusion Treating unstable humerus surgical neck fractures with locking plates combined minimally invasive surgery, has advantages that less surgical trauma and bleeding, stable fixing, quick fracture healing and recovery of functional upper limbs,fewer complications. This treatment is an ideal method.
