摘要
目的:观察固本调免方治疗气阴两虚型Ig A肾病的临床疗效。方法:将63例Ig A肾病患者随机分为2组;治疗组32例给予固本调免方治疗,对照组31例给予固本通络方治疗,疗程2个月。治疗结束,评价临床疗效及相关免疫学指标。结果:1治疗组总有效率为87.5%,对照组为61.3%,组间疗效比较有统计学差异(P<0.01);2治疗后,两组尿红细胞数目降低(P<0.05,P<0.01),治疗组尿红细胞数目显著低于对照组(P<0.01);两组24h尿蛋白定量显著降低(P<0.05,P<0.01),且治疗组低于对照组(P<0.05)。3治疗后,治疗组外周血免疫球蛋白Ig G、Ig A及补体C3有显著改善(P<0.01),且治疗组Ig G、Ig A、C3改善优于对照组(P<0.05,P<0.01);治疗组CD4显著升高(P<0.01)、CD8显著降低(P<0.01)、CD4/CD8比值显著上升(P<0.01),且两组间CD4、CD8、CD4/CD8比值有统计学差异(P<0.01)。结论:固本调免方治疗气阴两虚型Ig A肾病临床疗效确切,其机制与改善患者体液免疫和细胞免疫有关。
Objective: To observe the clinical efficacy of'Guben Tiaomian Formula'in treating Ig A nephropathy of both qi and yin deficiency pattern. Methods: Sixty-three cases with Ig A nephropathy were randomized into two cases: treatment group in which 32 cases were treated by'Guben Tiaomian Formula',and control group in which 31 cases were treated by'Guben Tongluo Formula',with the course of 2 months. After treatment,the clinical efficacy and related immunological indexes were evaluated.Results: 1 The overall effective rate was 87. 5% in the treatmentgroup and 61. 3% in the control group,with a difference between the two groups( P < 0. 01); 2 After treatment,urinary RBC count was reduced in both groups( P < 0. 05,P < 0. 01),urinary RBC count in the treatment group was less than that of the control group( P < 0. 01); 24 h urinary protein excretion was reduced significantly in both groups( P < 0. 05,P < 0. 01),treatment group was lower than that of the control group( P < 0. 05).3 After treatment,Ig G and Ig A in peripheral blood and C3 were increased in the treatment group( P < 0. 01),and were higher that those of the control group( P < 0. 05,P < 0. 01); CD4 was increased in the treatment group( P < 0. 01) and CD8 was reduced( P < 0. 01). The ratio of CD4 / CD8 was increased remarkably( P < 0. 01),with significant difference in CD4,CD8 and CD4 / CD8 between the two groups( P < 0. 01). Conclusion: 'Guben Tiaomian Formula'has positive effect on Ig A nephropathy of both qi and yin deficiency pattern. Its mechanism may be association with the humoral immunity and cell-mediated immunity.
