摘要
目的:观察析健脾化瘀汤对胃癌患者化疗前后免疫功能及毒副反应影响。方法:选取2016年1月~2018年1月间在本院接受治疗的脾虚痰瘀型晚期胃癌患者96例,随机数字表法分成对照组(48例)行FOLFOX4方案化疗,观察组(48例)在对照组基础上加服健脾化瘀汤;观察患者临床疗效及不良反应状况,流式细胞仪检测血清T淋巴细胞亚群(CD4+、CD3+、CD8+)含量,ELISA法检测血清sEC、CEA及CA19-9含量,采用卡氏体力(KPS)量表评价患者治疗前后形态状态情况,欧洲癌症研究与治疗组织编写的EORTC生命质量检测量表(QLQ-C30)检测患者治疗前后生存质量状况。结果:观察组患者总的有效率为56.25%,对照组为41.67%,差异有统计学意义(P<0.05);治疗后对照组患者血清CD4+、CD3+及CD8+含量比观察组、治疗前下降,差异均有统计学意义(P<0.05);观察组患者中医证候总有效率为81.25%,对照组为60.42%,差异有统计学意义(P<0.05);治疗后患者KPS评分及QLQ-C30评分比治疗前、对照组升高,差异有统计学意义(P<0.05);治疗后观察组出现血肌酐升高、白细胞降低、丙氨酸氨基转移酶升高、周围神经异常及血小板下降等几率均低于对照组,差异有统计学意义(P<0.05)。结论:健脾化瘀汤结合化疗对胃癌晚期患者疗效显著,可提高患者机体免疫功能,降低化疗产生的毒副反应。
Objective:To analyze the effect of Jianpi Huayu Decoction on immune function and toxicity for patients with gastric cancer before and after chemotherapy.Methods:Ninety-six patients with advanced gastric cancer of the spleen-deficiency type who were treated in our hospital from January 2016 to January 2018 were enrolled.The patients were randomly divided into controlled group(48 patients)who underwent FOLFOX4 chemotherapy and observation group(48 patients).On the basis of the controlled group,the Jianpi Huayu Decoction was added,the clinical efficacy and adverse reaction status of the patients were observed,and the serum T lymphocyte subsets(CD4+,CD3+,CD8+)were detected by flow cytometry.Serum sEC and CEA were detected by ELISA.And CA19~9 content,the Karst force(KPS)scale was used to evaluate the morphological status of patients before and after treatment.The EORTC Quality of Life Test Scale(QLQ-C30)prepared by the European Cancer Research and Treatment Organization tested the quality of life of patients before and after treatment.Results:The total effective rate was 56.25%in the observation group and 41.67%in the controlled group.The difference was statistically significant(P<0.05).The serum CD4+,CD3+and CD8+levels in the controlled group were lower than those in the observation group and before treatment.Statistically significant(P<0.05).The total effective rate of TCM syndrome was 81.25%in the observation group and 60.42%in the controlled group,the difference was statistically significant(P<0.05).KPS score and QLQ-C30 score after treatment compared with the controlled group and the controlled group,the difference was statistically significant(P<0.05).After treatment,the observed group showed increased serum creatinine,leukopenia,elevated alanine aminotransferase,peripheral nerve abnormalities and thrombocytopenia.All were lower than the controlled group,and the difference was statistically significant(P<0.05).Conclusion:Jianpi Huayu Decoction combined with chemotherapy has a significant effect for patients with
