摘要
目的:观察中医三焦合脏腑辨证法在治疗外感发热患者中的应用效果。方法:选取2012年2月~2018年3月外感发热患者106名,按照随机数字表分为观察组与对照组各53例,对照组给予西医解热镇痛、止咳祛痰、缓解咽喉肿痛等对症治疗,观察组采用中医三焦合脏腑辨证法治疗,连续治疗5天,观察两组中医症候积分变化、两组疗效、血清炎性因子[白细胞介素-1(IL-1)、肿瘤坏死因子-α(TNF-α)及巨噬细胞炎症蛋白-1(MIP-1)]水平,并观察两组不良反应。结果:治疗后,两组发热、恶寒、鼻塞流涕、咽喉肿痛、全身不适中医症候积分均显著低于治疗前(P<0.05),且观察组下降幅度大于对照组(P<0.05);观察组总有效率98.11%,显著高于对照组的84.91%(P<0.05);两组血清IL-1、TNF-α、MIP-1水平均显著低于治疗前(P<0.05),且观察组改善幅度大于对照组(P<0.05);观察组不良反应总发生率3.77%,显著低于对照组的22.64%(P<0.05)。结论:中医三焦合脏腑辨证法用于外感发热患者治疗中,可以缓解中医症候,提高临床疗效,改善血清与发热相关的炎性因子水平,且不良反应少。
Objective:To observe the application effect of TCM triple energizer combined with viscera syndrome differentiation method in treating patients with exogenous fever.Methods:106 cases of patients with exogenous fever from February 2012 to March 2018 were selected and divided into observation group and controlled group with 53 cases in each group according to the random number table method.The controlled group was given western medicine antipyretic analgesia,cough expectoration,sore throat relief and other symptomatic treatments,and the observation group was treated with TCM triple energizer combined with viscera syndrome differentiation method,and they were treated continuous for 5 days.The TCM symptoms scores,the efficacy,and the levels of serum inflammatory factors[interleukin-1(IL-1),tumor necrosis factor-α(TNF-α)and macrophage inflammatory protein-1(MIP-1)]were observed in the two groups,and the adverse reactions were observed in the two groups.Results:After treatment,the TCM syndromes scores of fever,aversion to cold,stuffy nose,sore throat and general malaise in the two groups were all significantly lower than those before treatment(P<0.05),and the decrease in the observation group was greater than that in the controlled group(P<0.05).The total effective rate in the observation group was significantly higher than that in the controlled group(98.11%vs 84.91%)(P<0.05).The levels of serum IL-1,TNF-αand MIP-1 in the two groups were significantly lower than those before treatment(P<0.05),and the improvement in the observation group was greater than that in the controlled group(P<0.05).The overall incidence rate of adverse reactions in the observation group was significantly lower than that in the controlled group(3.77%vs 22.64%)(P<0.05).Conclusions:TCM triple energizer combined with viscera syndrome differentiation method for patients with exogenous fever can relieve TCM symptoms,improve clinical efficacy,and promote serum levels of inflammatory factors associated with fever,and it has few adverse reactions.
