小复方清泄郁热饮在呼吸道感染疾病中退热作用临床观察

Clinical Observation on the Antipyretic Effect of Xiaofufangqingxieyure Decoction in Respiratory Tract Infections

摘要 目的观察小复方清泄郁热饮对呼吸道感染疾病所致外感发热的退热作用,为中药治疗外感发热提供理论依据。方法选取2017-05/2018-05月作者医院肺病科病房由呼吸道疾病所致的外感发热患者120例,随机分为观察1组40例,观察2组40例,对照组40例,观察并比较分析各组退热效果、中医临床疗效、炎症生化指标、致炎因子水平改善情况。结果经治疗后各组体温变化显示,观察1组、观察2组、对照组3组在各时间点内患者体温变化均有统计学意义(F值依次为1149365.420、1843227.100、1966367.020,P值依次为0.000、0.000、0.000);在观察各时间段内各组体温变化显示在2 h内3组体温变化有统计学差异(F=5.201,P=0.008),且对照组退热效果优于观察1组、观察2组(P值依次为0.009、0.041);在4 h内3组体温变化有统计学差异(F=4.047,P=0.021),且对照组退热效果优于观察2组(P=0.032);48 h、72 h内3组体温变化有统计学差异(F值依次为8.414、68.795,P值依次为0.000、0.001),且48 h、72 h内对照组退热效果差于观察1组、观察2组(48 h:P值依次为0.005、0.003;72 h:P值依次为0.014、0.002)。其余时间点3组体温变化无统计学差异;中医临床疗效改善方面结果显示观察1组、观察2组疗效明显好于对照组(Z值依次为-3.071、-2.284,P值依次0.002、0.005);3组经治疗后观察1组、观察2组肿瘤坏死因子α(tumor necrosis factor alpha,TNF-α)、白细胞介素1(interleukin 1,IL-1)、IL-6较对照组下降明显,观察1组、观察2组之间无统计学差异;治疗后3组白细胞(white blood cell,WBC)计数、降钙素原(procalcitonin,PCT)、超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)等炎症指标改善方面观察1、2组优于对照组,观察1组、观察2组之间无统计学差异。结论小复方清泄郁热饮在呼吸系统疾病所致外感发热的治疗过程中临床疗效显著且副作用少,值得临床推广。 Objective To investigate the antipyretic effect of Xiaofufangqingxieyure decoction on diseases caused by respiratory tract infections,and to provide theoretical basis for the traditional Chinese medicine(TCM)treatment of exogenous fever.Methods A total of 120 patients with fever caused by respiratory diseases were randomly divided into observation group 1 and 2(each 40 cases)and control group(40 cases)from May 2017 to May 2018.The antipyretic effect,clinical efficacy of TCM,the improvement of inflammatory biochemical indicators and inflammatory factors were observed and compared.Results After treatment,the changes of body temperature in each group showed that the changes of body temperature at each time point in observation group 1,group 2 and the control group were significantly different(F=1149365.420,1843227.100,1966367.020;P=0.000,0.000,0.000).The results showed that there were significantly different in body temperature among the three groups within 2 hours(F=5.201,P=0.008),and the antipyretic effect of the control group was better than that of the observation group 1 and 2(P=0.009,0.041).The temperature changes of the three groups within 4 hours were significantly different(F=4.047,P=0.041),and the antipyretic effect of the control group was better than that of the observation group 2(P=0.032).The temperature changes of the three groups within 48 hours and 72 hours were significantly different(F=8.414,68.795;P=0.000,0.001),and the antipyretic effect of the control group was worse than that of the observation group 1 and 2 within 48 hours and 72 hours(48 h:P=0.005,0.003;72 h:P=0.014,0.002).There was no significant difference in body temperature among the three groups at other time points.The results showed that the curative effect of observation group 1 and observation 2 was significantly better than that of control group(Z=-3.071,-2.284;P=0.002,0.005).The levels of tumor necrosis factor alpha(TNF-a),interleukin 1(IL-1),IL-6 of the observation group 1 and 2 were significantly lower than those of control grou
