摘要
目的:观察扶正祛瘀汤同步放疗治疗胃癌根治术后腹腔淋巴转移癌患者的临床治疗效果及对IL-4、IL-10及TNF-α水平的影响。方法:选取胃癌根治术后腹腔淋巴转移癌患者80例,随机分为对照组及观察组,对照组39例,观察组41例。对照组患者采用放疗,而观察组在放疗的基础上同步加用扶正祛瘀汤治疗2疗程。观察比较治疗前后2组患者血清中的炎性因子肿瘤坏死因子-α(TNF-α)、白介素-4(IL-4)和白介素-10(IL-10)的变化;比较治疗后2组生存质量评分,无进展生存期(RFS)和总生存期(OS),临床疗效及不良反应。结果:2组患者治疗后血清中炎性因子TNF-α、IL-4及IL-10较治疗前显著改善(P<0.05),并且观察组较对照组变化更为明显(P<0.05)。治疗后观察组患者生存质量评分的功能领域、症状领域及其他均明显优于对照组(P<0.05);观察组患者1年PFS率及OS率与对照组患者相比,差异有统计学意义(P>0.05)。观察组的临床疗效明显优于对照组(P<0.05),不良反应明显少于对照组(P>0.05)。结论:扶正祛瘀汤同步放疗治疗胃癌根治术后腹腔淋巴转移癌具有良好的临床疗效,值得在临床上推广。
Objective: To observe the effects of using Fuzheng Quyu Decoction concurrent radiotherapy in the treatment of lymph node metastasis carcinoma after radical gastrectomy for gastric cancer and its influence on the levels of IL-4,IL-10 and TNF-α. Methods: 80 cases of cancer patients with abdominal lymph node metastasis after radical resection of gastric cancer,were randomly divided into controlled group and observation group,the control group with 39 cases,the observation group with 41 cases.2 patients were treated with radiotherapy,and the observation group in radiotherapy based on the simultaneous use of Fuzheng Quyu Decoction,2 groups were treated continuously for 6 courses. Inflammatory cytokines tumor necrosis factor-αwere compared between the 2 groups before and after treatment in patients with serum alpha(TNF-alpha),interleukin-4(IL-4) and interleukin-10(IL-10) changes; Comparison of the quality of life scores of the 2 groups after treatment,progression free survival(RFS)and overall survival(OS),the clinical efficacy and adverse reactions. Results: 2 groups of patients after treatment of serum inflammatory factor TNF-alpha,IL-4 and IL-10 treatment significantly improved(P<0. 05),and the observation group than in the controlled group were more significantly(P<0. 05). The function field of the quality of life of patients with scores of the observation group after treatment,symptoms and other areas were significantly better than the controlled group(P<0. 05); The patients in the observation group were 1 years PFS rate and OS rate compared with the controlled group,the difference was statistically significant(P>0. 05). The clinical curative effect of the observation group was significantly better than the controlled group(P <0. 05),the adverse reaction was significantly less than the controlled group(P > 0. 05). Conclusion: The radical operation of Fuzheng Quyu Decoction after radiotherapy in the treatment of gastric cancer peritoneal metastases has a good clinical effect,worthy of clinical popularization.
