摘要
目的:观察助卵养巢方联合西药治疗对多囊卵巢综合征(PCOS)患者卵巢储备功能及经期紊乱的影响。方法:将我院妇产科收集的84例PCOS不孕患者根据入院就诊时间随机分为中药组、西药组和联合用药组,每组28例,中药组服用"助卵养巢"方,西药组服用橼酸氯米芬胶囊,联合用药组采用"助卵养巢"方联合橼酸氯米芬胶囊治疗。治疗2个疗程后,观察各组患者临床疗效、血清中促卵泡激素(FSH)、促黄体激素(LH)、睾酮(T)、基础抗苗勒管激素(AMH)等激素变化情况、月经改善及不良反应发生情况。结果:联合用药组治疗总有效率(92.85%)显著高于中药组(67.86%)及西药组(75.0%),差异具有统计学意义(P<0.05)。治疗后三组患者血清中LH、LH/FSH、T和AMH较治疗前显著降低(P<0.05),且与中药组和西药组比较,联合用药组中LH/FSH、T和AMH均显著降低(P<0.05)。联合用药组在月经改善和半年内妊娠率均显著高于中药组和西药组(P<0.05)。全部患者在服药期间均未出现任何严重不良反应。结论:助卵养巢方联合西药治疗PCOS能够明显改善患者临床症状,能够有效降低LH、T、AMH等激素水平,从而改善卵巢多囊性症状,恢复月经周期,提高排卵率和妊娠率,其疗效要优于单独中药或西药治疗,且安全性高。
Objective: To explore the effect of Zhuluan Yangchao Decoction combined with western medicine on ovarian reserve function and menstrual disorders for patients with polycystic ovary syndrome( PCOS). Methods: Eighty-four infertile patients with PCOS in our hospital were randomly divided into the traditional Chinese medicine( TCM) group,western medicine group and the combined medication group according to the time of admission,28 cases in each group. The TCM group was treated with Zhuluan Yangchao Decoction,the western medicine group was treated with Clomiphene Citrate Capsules,and the combined medication group were treated with Zhuluan Yangchao Decoction combined with Clomiphene Citrate Capsules. After 2 courses of treatment,the clinical efficacy,changes of serum follicle stimulating hormone( FSH),luteinizing hormone( LH),testosterone( T) and anti-Mullerian hormone( AMH),improvement of menstruation and adverse reactions were observed. Results: The total effective rate of treatment in the combined medication group( 92. 85%) was significantly higher than that in the TCM group( 67. 86%) and the western medicine group( 75. 0%)( P<0. 05). After treatment,the levels of serumLH,LH/FSH,T and AMH in the three groups were significantly lower than those before treatment( P<0. 05),and the levels of LH/FSH,T and AMH in the combined medication group were significantly lower than those in the TCM group and the western medicine group( P <0. 05). The improvement of menstruation and the pregnancy rate within six months in the combined medication group were significantly higher than those in the TCM group and western medicine group( P<0. 05). There were no serious adverse reactions during the treatment. Conclusion: Zhuluan Yangchao Decoction combined with western medicine can significantly relieve the clinical symptoms and effectively reduce the levels of LH,T,AMH and other hormones,so as to relieve the polycystic symptoms of ovary,restore menstrual cycle,and improve the ovulation rate and pregnancy rate.
