摘要
目的:观察活血壮骨汤联合洛索洛芬钠治疗膝关节骨性关节炎临床疗效,分析中西结合治疗对患者步态特征以及平衡能力的影响。方法:选取我院骨科收治的92例膝关节骨性关节炎患者为研究对象,采用随机数字表分为对照组(n=46例)与观察组(n=46例),患者入院后均给以消炎、镇痛及功能锻炼等基础治疗,对照组在基础治疗上予以洛索洛芬钠,观察组同时口服中药活血壮骨汤,记录两组患者临床疗效和治疗前后平衡能力及步态的改善情况,并观察两组不良反应。结果:观察组总有效率(89.13%)明显高于对照组(69.57%),差异具有统计学意义(P<0.05);治疗后观察组各项稳定指数明显优于对照组,差异具有统计学意义(P<0.05);观察组步长、步速显著高于治疗前,双支撑相显著低于治疗前,且治疗后观察组患者的步长明显长于对照组,差异具有统计学意义(P<0.05);观察组不良反应发生率为4.35%,明显低于对照组32.61%,差异具有统计学意义(P<0.05)。结论:活血壮骨汤结合功能锻炼、西药治疗能促进膝关节骨性关节患者平衡能力和步态改善,且并未增加不良反应,具有较高的安全性,临床可予以考虑。
Objective: To observe the clinical effect of Huoxue Zhuanggu Decoction combined with loxoprofen sodium for the treatment of knee osteoarthritis,and to analyze the effect of Chinese and western combination therapy on gait characteristics and balance ability of patients. Methods: 92 cases of knee osteoarthritis patients treated in the department of orthopedics in our hospital were selected for the study and divided into controlled group( n = 46) and observation group( n = 46) according to the random number table method. The patients were given basic anti-inflammatory and analgesic and functional exercise therapy,and the controlled group was given loxoprofen sodium based on the basic treatment,and the observation group was given Huoxue Zhuanggu Decoction. The clinical curative effect and the improvement of balance and gait before and after treatment were recorded and the adverse effects were observed. Results: The total effective rate was 89. 13% in the observation group,which was significantly higher than that in the controlled group with 69. 57%,and the difference was statistically significant( P<0. 05). After treatment,the stability index of the observation group was significantly better than that of the control group( P<0. 05). The pace of the observation group was significantly higher than that before treatment,and the two-step phase was significantly lower than that before treatment,and the pace of the observation group was significantly longer than that of the controlled group( P< 0. 05). The incidence rate of adverse reactions was 4. 35% in the observation group,which was significantly lower than that in the controlled group with 32. 61%( P<0. 05). Conclusion: Huoxue Zhuanggu Decoction combined with functional exercise,western medicine treatment can promote balance ability and gait improvement without increasing the adverse reactions and with high safety in patients with knee joint osteoarthritis,therefore it can be considered in clinical application.
