摘要
目的:研究中央型小细胞肺癌CT征象分布与中医辨证分型之间的规律及关系。方法:以我院2011年1月~2015年11月所收集120例中央型未分化小细胞肺癌患者为例进行研究,均行CT检查并进行中医辨证分型,观察并记录患者阻塞性肺炎、阻塞性肺不张、分叶征、恶性胸水、心包积液、毛刺征、空洞形成、阻塞性肺气肿、侵犯血管、侵犯椎体等CT征象,统计不同证型患者中相关CT征象出现情况。结果:120例患者中40例为肺气虚型、34例痰浊壅肺型、16例肺肾两虚型、12例气阴虚型、10例气滞血瘀型、8例肺阴虚型,以肺气虚型及痰浊壅肺型为主。在CT征象分布方面,以阻塞性肺炎、阻塞性肺不张、分叶征、恶性胸水、心包积液居多,其中阻塞性肺炎、阻塞性肺不张、分叶征、恶性胸水都主要出现于痰浊壅肺及肺气虚型患者中,心包积液征象则以气滞血瘀、肺气虚、痰浊壅肺为主。结论:中央型小细胞肺癌CT征象分布与中医辨证分型之间存在一定规律,二者联合利于提高辨证分型的客观性,可为后续治疗针对性的提高奠定基础,也可拓宽现代影像学领域,值得深入研究。
Objective: To study the regularity and relationship between the CT signs of central type small cell lung cancer and the differentiation of TCM syndromes. Methods: From January 2011 to November 2015,120 cases of central type of undifferentiated small cell lung cancer patients were studied. All patients were detected with CT. The CT signs were observed and recorded in patients with obstructive pneumonia,obstructive pulmonary edema,pleural effusion,malignant pleural effusion,pericardial effusion,spicule sign,void formation,obstruction and pulmonary emphysema. Results: 40 cases of Qi deficiency,34 cases of phlegm obstructing lung type,16 cases of lung and kidney deficiency type,12 cases of Qi Yin deficiency type,10 cases of Qi stagnation and blood stasis,8cases of lung Yin deficiency,with Qi deficiency and phlegm type. The distribution of CT signs,with obstructive pneumonia,atelectasis,lobulation,pleural effusion and pericardial effusion were among the obstructive pneumonia,atelectasis,lobulation,pleural water mainly occurs in the phlegm and Qi deficiency in patients with signs of pericardial effusion the lung Qi deficiency,Qi stagnation and blood stasis,phlegm. Conclusion: There is a certain regularity between the distribution of CT signs in the central type small cell lung cancer and TCM syndrome differentiation,and the two is to improve the objectivity of the syndrome differentiation. It can lay the foundation for the improvement of the follow-up treatment.
