PDCA管理在时间依赖性抗菌药物输注中的应用 被引量:1

摘要 目的:调查时间依赖性抗菌药物在临床护理输注中的现状,分析存在的问题,提出相应护理对策,观察整改后效果。方法:采用PDCA (计划-实施-检查-处理)循环管理模式,随机抽查我院感染科2016年2月至7月(Group1、G1)期间Q8h输注的时间依赖性抗菌药物768例共2304组(PDCA实施前组),在规定时间点±30分钟内输注为合格。分析不合格原因,然后采取相应护理对策。再随机抽查2016年9月至2017年2月(Group2、G2)期间的852例共2556组(PDCA实施后组)。统计出三个时间点的合格率,并与G1期间相应时间点的数据进行统计学分析。结果:G1期间9点合格率为98.70%,17点合格率为94.79%,1点合格率为89.71%。不合格输注原因包括护士及患者主观原因和客观原因三方面。采取护理PDCA对策后,G2期间9点合格率为100%,17点合格率为99.77%,1点合格率为99.30%。采取护理PDCA对策前后各时间点的输注合格率之间具有统计学差异(P<0.05)。结论:时间依赖性抗菌药物输注中存在较多问题,PDCA循环管理模式能提高时间依赖性抗菌药物输注的合格率,保证抗菌疗效。 Objective:To investigate the current situation of time dependent antimicrobial agents in clinical nursing infusion, analyze the existing problems, put forward the corresponding nursing countermeasures, and observe the effect of rectification. Methods:Application of PDCA(plan-Implementation-check-processing) cycle management model, randomly selected 768 cases in a total of 2304 groups(PDCA before the implementation of the group) of Q8 h infusion time dependent antimicrobial agents during February to July of 2016(Group1, G1). the infusion within 30 minutes of the specified time is qualified. Analysis of the reasons for failure, and then take the appropriate care measures. And then randomly selected 852 cases in a total of 2556 groups(PDCA after the implementation of the group) during September 2016 to February 2017(Group2, G2). Statistics the qualified rate, and compared with G1 data. Results:the qualified rate in G1: 98.70% for 9 am, 94.79% for 5 pm, 89.71% for 1 am. The causes of unqualified infusion include three aspects: subjective reasons(nurses and patients) and objective reasons. While the qualified rate in G2: 100% for 9 am, 99.77% for 5 pm, 99.30% for 1 am after nursing PDCA. There was a significant difference in the pass rate of each time point before and after nursing PDCA(P<0.05). Conclusion:There are many problems in the infusion of time dependent antibiotics, PDCA cycle management model can improve the qualified rate of time dependent antibiotic infusion, to ensure the efficacy of antibiotics.
