慢性肾功能不全患者血浆血管紧张素Ⅱ、神经肽Y、神经降压素和甲状旁腺激素的变化及其临床意义 被引量:3

摘要 目的 探讨慢性肾功能不全患者血浆血管紧张素Ⅱ (AngiotensinⅡ ,AngⅡ )、神经肽Y(NeuropeptideY ,NPY)、神经降压素 (Neurotensin ,NT)全段甲状旁腺激素 (Intactparathyroidhormone ,iPTH)浓度变化及其临床意义。方法 采用放射免疫方法 (Radioimmunoassay ,RIA)检测血浆AngⅡ、NPY、NT浓度 ,采用免疫放射方法 (Immunoradiometricassay,IRMA)检测血清iPTH。健康对照组 (A组 ) 2 0例 ,肾功能不全代偿期 (B组 ) 2 0例 ,氮质血症期 (C组 ) 2 0例 ,尿毒症期 (D组 ) 2 0例。结果 随肾功能衰竭程度日趋加重 ,AngⅡ浓度依次升高 ,C、D组与A组相比 ,有显著性差别 (P <0 .0 0 1) ;NPY浓度逐渐增加 ,B、C、D组与A组相比 ,有统计学意义 (P <0 .0 5 ) ;NT浓度依次降低 ,D组与A组相比有显著性差异 (P <0 .0 0 1) ;iPTH浓度逐渐升高 ,D组与A组相比 ,有显著性差别 (P <0 .0 0 1) ;AngⅡ、NPY、iPTH与Ccr呈负相关 ,NT与Ccr呈正相关 ;AngⅡ与NT呈负相关。结论 增高的AngⅡ、NPY、iPTH和降低的NT在CRF进展中起重要作用 ,为CRF的预防和治疗提供理论依据。 Objective To explore the changes and clinical significance of the plasmic angiotensinⅡ (AngⅡ), neuropeptide Y (NPY), neurotensin (NT) and intact parathyroid hormone (iPTH) in patients with chronic renal insufficiency. Methods The plasmic AngⅡ、NPY and NT were measured by radioimmunoassay (RIA). The serum iPTH were measured by immunoradiometric assay (IRMA).There were four groups and twenty cases of each as group, A,B, C and D. They were normal controls, chronic renal insufficiency compensated stage, azotemia stage and uremia stage respectively.Results With the severity of renal failure,the level of AngⅡ showed an ascending tendency. Groups C and D were higher than normal control group (P<0.001); The concentration of NPY (groups B.C and D) were higher than group A (P<0.05); The level of NT showed a decending tendency, group D were lower than group A(P<0.001); The iPTH (group D) were significant higher than group A (P<0.001). In addition to the negative correlation between AngⅡ or NPY or iPTH and Ccr, the negative correlation between NT and Ccr, negative correlation between AngⅡ and NT were found.Conclusion The increased AngⅡ、NPY、iPTH and decreased NT play important roles in the further decrease of renal function and also provide theoretical bases for chronic renal failure's preventions and treatments.
