摘要 目的:观察妊娠肝内胆汁淤积症(ICP)孕鼠肝细胞超微结构变化和表皮生长因子受体(EGFR)及其mRNA的表达,探讨ICP的发病机制。方法:用电镜观察孕晚期接受不同剂量苯甲酸雌二醇(EB)的清洁级孕鼠孕足月和分娩1周后肝细胞超微结构变化;应用免疫组化法和逆转录-聚合酶联反应(RT-PCR)技术对肝细胞EGFR/EGFR mRNA表达进行分析。结果:注射不同剂量雌激素后,孕鼠肝细胞超微结构出现不同程度改变,核内异染色质异常分布,滑面内质网增多、扩张,线粒体肿胀、嵴断裂、消失,胞质出现空泡,毛细胆管扩张、胆汁淤积等。随雌激素剂量增加变化更显著;分娩1周后,肝细胞超微结构基本恢复正常。实验组足月孕鼠肝细胞 EGFR/EGFR mRNA的表达均随雌激素剂量的增加而降低,与对照组相比,P<0.05(除外低EB组,P>0.05)。结论:雌激素可引起孕鼠肝内胆汁淤积、肝细胞超微结构可逆性改变;肝细胞EGFR及其mRNA表达降低,并呈一定剂量依赖关系。 Objective: To study the changes of ultrastructure and the expressions of epidermal growth factor receptor (EGFR) and EGFR mRNA in the hepatocyte of pregnant rats with intrahepatic cholestasis and explore its pathogenesis. Methods: The ultrastructure of hepatocytes in pregnant rats induced by different concentrations of estradiol benzoas (EB) was observed before and after one week of parturition.The expression of EGFR and its mRNA in hepatocyte was analyzed by immunohistochemical method (ABC) and RT-PCR method respectively. Results: The ulterastructural changes of hepatocytes of intrahepatic cholestasis of pregnancy (ICP) in rats including abnormal distribution of heterochromosome, increase and dilatation of smooth endoplasmic reticulum, mitochondria swelling and loss of its ridge, dilation of the bile canaliculi, was variant with different concentrations of EB. The degree of these changes depended on the dose of EB. After delivery, these features were significandy ameliorated.The expressions of EGFR/EGFR mRNA in hepatocyte with ICP was negtively correlated with the dose of EB.There was a significant difference between experiment group and the control (P<0.05)apart from the group of low dose of EB(P>0.05). Conclusions: Etrogenic hormones may induce intrahepatic cholestasis and reversible change of ultrastructure in hepatocyte. The expression of EGFR/EGFR mRNA is significantly low in the experimental groups compared with the control groups. All of the changes of ICP depend on the dose of EB.
