摘要
目的探究儿童疑难配血的产生原因及处理措施。方法选择某儿童医院2014年2月—2016年3月各科室接诊的48例疑难配血患儿,进行血型鉴定、抗体筛查、交叉配血实验,并对实验数据进行总结分析。结果在48例儿童疑难配血标本中,2(4.2%)例为血型正、反定性不符儿童; 1例(2.1%)为ABO亚型; A型Rh阳性检测出抗-A抗体患儿2例(4.2%)。在对48例儿童的抗体筛查及鉴定中,(1) Rh血系统:2(4.2%)例儿童检出抗-D抗体; 6(8.4%)例儿童检出E抗体; 2(4.2%)例儿童检出c E抗体; 1例儿童检出抗c抗体; 8(16.8%)例儿童检出c E+自身抗体;自身Ig G抗体7(14.6%)例; 11(22.9%)例儿童检出自身冷抗体;(2) ABO血型系统:2(4.2%)例儿童检出抗-A抗体; 1(2.1%)例儿童检出抗-A1抗体;(3) MNS血型系统:35例儿童检出M抗体。结论不规则抗体抗M抗体、自身Ig G抗体和自身冷抗体等导致的儿童交叉配血不合是常见原因。在临床输血中,应注重输血的安全性、有效性,对儿童输血疑难原因进行科学分析,以确保输血的成功。
Objective To explore the cause of difficult blood match and treatment measures for children.Methods 48 children who were difficult to match blood receipted by various departments of a children’s hospital from February,2014 to March,2016 were enrolled in this study. Blood samples were collected for blood group identification,antibody screening,blood cross matching test,and the experimental data were analyzed.Results 2( 4.2%) of 48 samples collected from children with difficult blood-matching were inconsistent with positive and negative for blood group,one case( 2.1%) of ABO subtype. Anti-A antibody was found in 2( 4.2%) children with Rh positive A type blood.In the antibody screening and identification of Rh blood system,anti-D antibody was found in 2( 4.2%) cases of children,E antibody was found in 6( 8.4%) cases of children,c E antibody was found in 2( 4. 2%) cases of children,anti-C antibody was found in one case of children,c E antibody plus autoantibody was found in 8( 16.8%) cases of children,autologous Ig G antibody was found in 7( 14.6%) cases,11( 22. 9%) cases of children with autologous cold antibody. In ABO blood group system,2( 4. 2%) cases of children with anti-A antibody,1( 2. 1%) case of children was positive in anti-A1 antibody. In MNS antigen system,35 cases of children with Ig M antibody. Conclusions The incompatibility of irregular antibodies,M antibodies,Ig G antibodies and autoantibody were the common causes of incompatibility in children. In clinical blood transfusion,clinician should pay attention to the safety of blood transfusion and effectiveness,scientifically the difficult reasons for children’s blood transfusion,in order to ensure the success of blood transfusion.
