应用多普勒超声肺动脉频谱参数法估测先心病肺动脉压力——与术中实测值对照 被引量:10

摘要 目的 探讨多普勒超声估测先心病肺动脉压力的可行性、准确性 ,寻求可估测肺动脉压力的肺动脉频谱参数指标。方法 对 3 0例先心病患者进行超声多普勒检查 ,测量 8项肺动脉频谱参数 ,将所测各参数与手术中测量的肺动脉压力进行对比 ,作统计学相关性分析。结果 肺动脉频谱参数中右室射血前期与右室射血时间比值 (RPEP/RVET)、心率矫正的右室射血前期 (C RPEP)、右室射血前期 (RPEP)、右室射血前期与肺动脉血流加速时间比值 (RPEP/AT)等与肺动脉压力呈良好线性正相关 ;右室射血时间 (RVET)、心率矫正的右室射血时间 (C RVET)、肺动脉血流加速时间 (AT)与肺动脉压力呈良好线性负相关。结论 用肺动脉频谱参数可以较准确地估算肺动脉压力。 Objective To evaluate the accuracy and feasibility of Doppler echocardiography for estimating the pulmonary artery pressure (PAP) in patients with congenital heart disease (CHD) and to find the available parameters.Methods Eight pulmonary flow indexes were analyzed in 30 CHD patients by Doppler echocardiography,correlation and linear regression analyses were performed to assess the relation between each pulmonary flow index and pulmonary artery pressure measured during operation.Results High linear positive correlations were found between PAP and RPEP/RVET,C-RPEP,RPEP,RPEP/AT; Good linear negative correlations were found between PAP and RVET,C-RVET,AT.Conclusion Pulmonary flow index can be used to evaluate PAP and the method is accurate.
