新生儿肺动脉频谱参数测量值稳定性探讨

摘要 目的 探讨连续测量不同心动周期次数的均值对右室射血前期时间(RPEP)、加速时间(AT)、射血时间(RVET)参数值稳定性的影响。方法 应用彩色多普勒心脏超声诊断仪测量上述参数时,分为单个心动周期和连续2、3、4和5个心动周期,取平均值共5组,均重复测量5次。计算各组变异系数、标准差、极差、均数95%可信区间,比较连续3个心动周期与5个心动周期的参数测量值。结果 RPEP、AT、RVET三个参数,连续3个心动周期与5个心动周期测量均值比较,两组间无明显差异(t值分别为:0.3324、0.8274、1.683,P值均〉0.05)。单个心动周期测量,95%可信区间范围、极差、标准差、变异系数均较大;连续3、4、5个心动周期均值,95%可信区间范围明显变窄,极差、标准差、变异系数也较小,而且基本接近。结论 上述参数测量,以连续3、4、5个心动周期取均值。变异度小、代表性好。连续测量3个心动周期与5个心动周期取均值,两种方法对参数值结果无影响。 Objective To evaluate influence of the average value of different cardiac cycles on the stability of the three parameters of right ventricular pre-ejection period (RPEP), acceleration time(AT), right ventrieular ejection time(RVET) in neonate. Methods Three parameters,repeatedly five times, were measured to evaluate the value of single cardiac cycle,and average values of 2,3,4,5 cardiac cycles in series by Doppler echocardiography. The standard deviation, range, variation coefficient, 95 % confidence interval were analyzed, the averse values of 3 cycles were compared with those of 5 cycles. Results There was no significant difference in RPEP,AT, RVET parameter between the average values of 3 cycles and those of 5 cycles( t =0.3324,0.8274,1.683 respectively,P 〉0.05).The 95 % confidence interval was narrow, standard deviation, range, variation coefficient were relatively small by means of average values of 3 ,4,5 cycles in series. Conclusion The degree of variation of RPEP, AT , RVET parameter at 3,4,5 cycles in series is small. It makes no difference to evaluate RPEP,AT,RVET parameter between the average values of 3 cycles and thoee of 5 cycles in series.
