摘要 目的 调查开放性骨折创面处理与创面感染的关系。方法 对88例骨科急性开放性创口于不同时间、不同阶段采样进行细菌培养。结果 未处理前,85.23%(75/88)的病例创面标本培养有细菌生长;刷洗后细菌培养,阳性率为32.95%(29/88);清创术后细菌培养,阳性率为17.05%(15/88)。发生创面感染18例,其中手术后医院感染13例。细菌种类以革兰阳性菌为主,主要为葡萄球菌属、微球菌属、肠球菌属、链球菌属等,其中金黄色葡萄球菌占首位。革兰阴性菌则以大肠埃希菌和假单胞菌属为主。结论 开放性骨折创面的初步清洗非常重要,能有效地减少细菌。开放性骨折创面感染主要是手术后的医院感染。 Objective To evaluate the relation of wound treatment and wound infection in open fracture. Methods Bacterial cultures were done in samples taken from 88 patients with acute open wounds in different time and phases in orthopedics department. Results The positive rates of bacterial culture were 85.23% (75/88), 32.95% (29/88), 17.05% (15/88) respectively in the untreated, scrubbed and debrided wounds. 18 cases have wound infections, 13 cases of whom were infected after operation. Staphylococcus was the most common bacteria, followed by Micrococcus, Enterococcus, Streptococcus , E. coli and Pseudomonas were major gram-negative bacteria. Conclusion The initial scrubbing wounds in open fracture were very important, which can efficiently diminish the numbers of bacteria, the infections of open fracture were mainly caused by postoperative nosocomial infections.
