摘要
目的分析2型糖尿病(T2DM)合并非酒精性脂肪肝(NAFLD)患者血清nesfatin-1水平与胰岛素抵抗(IR)的相关性。方法选取2014年2月—2015年2月永清县人民医院收治的T2DM患者60例,根据2010年《非酒精性脂肪性肝病诊疗指南》分为T2DM合并NAFLD患者30例(T2DM-NAFLD组);单纯T2DM患者30例(T2DM组),另选取同期本院体检健康者30例为对照(NC)组。比较3组受检者性别、年龄、病程、体质指数(BMI)、腰围(WC)、收缩压(SBP)、舒张压(DBP)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、空腹血糖(FPG)、空腹胰岛素(FIns)、HOMA-IR、HOMA-β、DI、Nesfatin-1水平。结果 3组受检者性别、年龄比较,两组患者病程比较,差异无统计学意义(P>0.05);T2DM-NAFLD组患者BMI、WC、SBP、DBP、TG、AST、ALT、GGT、FPG、HOMA-IR高于NC组,HDL-C、HOMA-β、DI、Nesfatin-1低于NC组,差异有统计学意义(P<0.05)。T2DM-NAFLD组WC、TG、AST、ALT、FPG、FIns、HOMA-IR高于T2DM组,HDL-C、DI、Nesfatin-1低于T2DM组,差异有统计学意义(P<0.05)。血清nesfatin-1与BMI、WC、SBP、FPG、ALT、TG、HOMA-IR呈负相关(r=-0.404、-0.438、-0.436、-0.433、-0.305、-0.497、-0.339,P<0.05),与HOMA-β、DI呈正相关(r=0.395、0.476,P<0.05)。WC、TG与nesfatin-1水平独立相关(r=0.985,r=0.971,P<0.05)。结论T2DM合并NAFLD患者血清nesfatin-1水平最低,nesfatin-1可能参与IR、T2DM及NAFLD的发生、发展。
Objective To investigate the relationship between serum nesfatin- 1 level and insulin resistance( IR) in patients with type 2 diabetes mellitus( T2DM) and nonalcoholic fatty liver disease( NAFLD). Methods A total of 60 T2 DM patients in the People's Hospital of Yongqing from February 2014 to February 2015 were selected,according to the 2010 < nonalcoholic fatty liver disease diagnosis and treatment guidelines > they were divided into T2DM- NAFLD group and T2 DM group,30 cases in each,and 30 cases health people were selected as control group( NC group). The sex,age,course of disease,body mass index( BMI),systolic blood pressure( SBP),diastolic blood pressure( DBP),( total cholesterol) TC,triacylglycerol( TG),high density lipoprotein cholesterol( HDL- C),low density lipoprotein cholesterin( LDL- C), aspartic transaminase( AST),alanine aminotransferase( ALT),fasting blood- glucose( FPG),fasting insulin( FIns),HOMA-IR,HOMA- β and glucose disposal index( DI),Nesfatin- 1 among the three groups were calculated. Results The sex,age among the three groups showed no significant differences,course of disease between the two groups of patients showed no significant differences( P > 0. 05). The levels of BMI,WC,SBP,DBP,TG,AST,ALT,GGT,FPG,HOMA- IR of the T2DM- NAFLD group were higher than the NC group( P < 0. 05); the levels of HDL- C,HOMA- β,DI,Nesfatin- 1 of the T2DM- NAFLD group were lower than the NC group( P < 0. 05). The levels of WC,TG,AST,ALT,FPG,FIns of the T2DM- NAFLD group were higher than the T2 DM group( P < 0. 05); the levels of HDL- C,DI,Nesfatin- 1 of the T2DM-NAFLD group were lower than the T2 DM group( P < 0. 05).
