摘要
目的:探究鼻外伤所致鼻缺损患者美容整形皮瓣修复及护理效果。方法:选择2017年1月-2018年6月于笔者医院就诊的174例鼻外伤所致的鼻缺损患者,经随机数表法分为对照组(n=87)和观察组(n=87)。两组患者均进行皮瓣修复,术后对照组给予常规护理,观察组进行个体化护理。术后1年,比较两组治疗效果、焦虑自评量表(SAS)、抑郁自评量表(SDS)评分,并且对两组患者皮瓣色泽、感觉功能及治疗满意度进行比较分析。结果:观察组美容整形有效率(96.55%)明显高于对照组(85.05%,P<0.05)。护理后,观察组SAS、SDS评分显著低于对照组,差异有统计学意义(P<0.05)。治疗1年后,观察组原发皮损无复发,皮瓣区域色泽与周边正常皮肤一致,无感觉障碍,切口缝线瘫痕不明显。对照组2例出现感觉障碍,7例缝线瘫痕明显。观察组治疗满意度(98.85%)明显高于对照组(86.21%,P<0.05)。结论:美容整形皮瓣修复能较好地修复外伤所致的鼻缺损,提高患者鼻腔功能,此外采用个性化护理可以改善患者负性情绪,具有较好的远期疗效和美学效果,值得在临床上广泛推广。
Objective To explore the effect of cosmetic and plastic flap repair and nursing care in patients with nasal defects caused by nasal trauma. Methods 174 patients with nasal defects caused by nasal trauma were enrolled in our hospital from January to February 2018, and were randomly divided into control group(n=87) and observation group(n=87). Skin flaps were repaired in both groups, but the control group received routine care,the observation group underwent individualized care.One year after surgery, the treatment outcomes, self-rating anxiety scale(SAS), and self-rating depression scale(SDS) scores were compared. At the same time, the color, sensory function and treatment satisfaction of the flaps of the two groups were compared and analyzed. Results The effective rate of cosmetic surgery in the observation group(96.55%) was significantly higher than that in the control group(85.05%, P<0.05). After the nursing, the SAS and SDS scores of the observation group were significantly lower than those of the control group(P<0.05). After 1 year of treatment, the primary skin lesions in the observation group did not recur, the color of the flap area was consistent with the surrounding normal skin, no sensory disturbance, and the scar of the incision was not obvious. In the control group, 2 patients had sensory disturbances and 7 patients had obvious suture scars. The treatment satisfaction of the observation group(98.85%) was significantly higher than that of the control group(86.21%, P<0.05). Conclusion The cosmetic plastic flap has better nasal defect repair effect and improves the nasal function of the patient. In addition, personalized nursing can improve the negative emotion of the patient, and has good long-term curative effect and aesthetic effect. It is worthy of clinical widely promoted.
