摘要
目的比较经尿道铥激光前列腺剜除术(ThuLEP)及经尿道前列腺电切术(TURP)在治疗良性前列腺增生症患者引起的下尿路症状中的临床效果,以及对术后性功能的影响。方法选自2016年4月至2017年10月新疆喀什地区第二人民医院收治的120例BPH维族患者,按照随机数字表法分为观察组及对照组,每组各60例。观察组采用ThuLEP,对照组采用TURP。结果观察组及对照组术后国际勃起功能指数问卷(IIEF-5)评分升高,两组间相比无统计学差异(P>0.05);两组术后男性性健康问卷射精功能障碍部分评分(MSHQ-EJD)均下降,两组间相比无统计学差异(P>0.05);两组间国际前列腺症状评分(IPSS)、最大尿流率(Qmax)、残余尿量(PVR)的差异均无统计学意义(P>0.05)。结论 ThuLEP治疗BPH的效果要更明显,更具安全性,而且对维族患者的性功能影响较小。
Objective To compare the clinical efficacies of transurethral thulium laser enucleation of the prostate(ThuLEP)and transurethral resection of the prostate(TURP)in the treatment of lower urinary tract symptoms in patients with benign prostatic hyperplasia(BPH)and to explore their effects on sexual functions.Methods A total of 120 Uyghur BPH patients treated during April 2016 and Oct.2017 were randomly divided into observation group(treated with ThuLEP)and control group(treated with TURP),with 60 cases in either group.Results The postoperative international index of erectile function 5(IIEF-5)score increased in both observation group(P=0.71)and control group(P=0.96),and the score was higher in the control group than in the observation group.The international prostate symptom score(IPSS)decreased in both groups,and it was lower in the observation group than in the control group,but there was no statistical difference 6 months after surgery(P=0.39).The maximum flow rate(Qmax)significantly increased in both groups(P=0.005 and 0.006),while the post-void residual urine volume(PVR)significantly decreased(P=0.001 and 0.006),and the improvements were greater in the observation group than in the control group,but there were no statistical differences between the two groups(P=0.87 and 0.86).Conclusion ThuLEP is safer and more effective in the treatment of BPH,and has less impacts on sexual function in Uyghur patients.
