摘要
目的:观察通腑泄浊法对腹腔高压患者的疗效以及对炎症因子的影响。方法:144例患者按住院的先后顺序,按照事先由随机数确定的随机分组方法,进入治疗组和对照组。对照组按治疗常规给予脏器支持治疗及针对原发病的治疗,治疗组在西药治疗原发病基础上,入组后即开始予通腑泄浊汤剂口服。比较两组有效率、病死率、腹腔内压水平、腹腔灌注压水平、C反应蛋白、APACHE II评分、血气分析指标、血乳酸以及炎症因子TNF-α、IL-6、IL-8表达水平。结果:治疗组有效率较对照组高,而死亡率低于对照组,差异有统计学意义;两组治疗后较治疗前腹腔内压明显下降(P≤0.05),治疗组较对照组改变更明显(P≤0.05);两组治疗后较治疗前腹腔灌注压明显上升(P≤0.05),治疗组较对照组改变更明显(P≤0.05);两组APACHE II评分、C反应蛋白治疗后较治疗前明显下降(P≤0.05),治疗组较对照组改变更明显(P≤0.05);两组治疗后较治疗前动脉血氧合指数上升、二氧化碳分压下降(P≤0.05),治疗组较对照组改变更明显(P≤0.05);两组治疗后较治疗前血乳酸下降(P≤0.05),治疗组较对照组改变更明显(P≤0.05);两组治疗后较治疗前炎症因子TNF-α、IL-6、IL-8表达下降(P≤0.05),治疗组较对照组改变更明显(P≤0.05)。结论:应用通腑泄浊法治疗腹腔高压有效,并能降低腹腔内压,改善组织灌流和氧供,其机理可能与抑制炎症因子TNF-α、IL-6、IL-8表达有关。
Objective:To observe the therapeutic effect of dredging Fu-organs and purging the turbid method on the treatment of intra-abdominal hypertension.Methods:One hundred and forty-four critical patients with intra-abdominal hypertension were randomly divided into treatment group and control group according to the order of hospitalization and the random grouping method determined by the random number at first.The treatment group was given dredging Fu-organs and purging the turbid traditional Chinese medicine on the basis of the treatment of western medicine.The effective rate,mortality and intra-abdominal pressure were compared between two groups.Results:The effective rate of the treatment group was higher than that of the control group,while the mortality rate was lower than that of the control group.The difference was statistically significant.After treatment,intra-abdominal pressure of two groups decreased significantly(P≤0.05).Compared with the control group,the change in treatment group was more obvious and the difference was statistically significant(P≤0.05).Conclusion:Dredging Fu-organs and purging the turbid method is effective in the treatment of intra-abdominal hypertension of critical patients and can reduce intra-abdominal pressure.
