摘要
目的探讨室性早搏(VPBs)负荷与住院患者心源性猝死(SCD)的相关性。方法回顾性分析2016年6月~2017年10月住院患者6370例的临床资料,根据是否发生SCD分为对照组、SCD组,采用logistic回归方法分析SCD发生的危险因素。根据检出的VPBs负荷,将检出VPBs的患者分为低负荷组(<10%)、中负荷组(10%~20%)、高负荷组(>20%)。采用logistic回归方法分析不同VPBs负荷与SCD发生的风险。采用单因素方差分析比较不同VPBs负荷组心电图指标及左心室射血分数(LVEF)差异。结果多因素logistic回归分析显示,年龄、急性冠脉综合征或心肌病、QTc间期、QRS宽度、VPBs、LVEF、脑钠肽是SCD发生的独立危险因素。在调整各项其他危险因素后,VPBs中负荷组、高负荷组发生的SCD的风险分别为低负荷组的1.410倍、2.429倍(p<0.05)。随着VPBs负荷增高,QTc间期、QRS宽度增大,LVEF减小,差异有统计学意义(p<0.05)。结论 VPBs负荷增高是住院患者发生SCD的独立危险因素。
Objective To investigate the relationship between the burden of ventricular premature beats(VPBs and inpatient sudden cardiac death(SCD). Methods A total of 6370 patients from June 2016 to October 2017 were retrospectively enrolled and were divided into control group and SCD group. The risk factors of SCD were analyzed by logistic regression method. The patients who were detected with VPBs were divided into low-burden group(<10%),middle-burden group(10% ~ 20%), and high-burden group(>20%) by the burden of VPBs. Logistic regression was used to analyze the risk of different burden of VPBs for SCD. One-way ANOVA analysis was used to compare the electrocardiogram indicators and the left ventricular ejection fraction(LVEF) in different VPBs burden groups.Results Multiple logistic regression analysis showed that age, acute coronary syndrome or cardiomyopathy diseases,QTc interval, QRS duration, VPBs, LVEF, BNP were independent risk factors of SCD. After adjusting other risk factors, middle-burden group and high-burden group of VPBs had higher risk of SCD compared with low-burden group(OR=1.410, 2.429 respectively, p<0.05). The QTc interval and QRS duration were significantly increased, while LVEF were significantly decreased with the increase of VPBs burden(p<0.05). Conclusions The burden of VPBs is an independent risk factor for inpatient SCD.
