摘要
目的审核妇幼医院静脉用药医嘱缺陷并实施干预,确保临床合理用药。方法根据审核时发现的医院2013年1月-2014年12月静脉用药缺陷医嘱,分析原因,实时与医护人员沟通后进行调配干预。结果审核中发现静脉用药缺陷医嘱1 684份,原因分别为录入错误、溶媒选择不当、配制浓度不适宜、剂量不当、配伍不当、特殊人群禁忌用药、重复用药及批次安排不当等,并在调配时予以纠正。结论药师审核静脉用药医嘱并及时干预医嘱缺陷,是促进临床合理用药的重要环节。
AIM To audit and intervene the defect prescriptions of intravenous medication in our hospital,and ensure the rational use of drugs. METHODS The defect prescriptions of intravenous medication were analyzed and intervened in our hospital from January 2013-December 2014. The defect prescriptions were classified by the reasons. After communicating with the physicians and nurses,the defect prescriptions were intervened for dispensing. RESULTS Totally 1 684 cases of the defect prescriptions were discovered. There were 8 types of errors in the defect prescriptions such as copy error,inappropriate solvents,inappropriate configuration concentration,dosage error,incompatibility,contraindicated use for special population,repeated use,and batch arrangement error. After audited by pharmacists,the above errors were corrected at the time of drug dispensing. CONCLUSION Pharmacists review and intervene intravenous medication timely,which may promote the rational use of drugs.
