摘要
目的探讨感染幽门螺杆菌(Hp)患者功能性消化不良(FD)发病及临床症状特点。方法选取上海市第八人民医院自2016年7月至2017年10月收治的140例Hp阳性患者为观察组,选取同期未检出Hp感染的100例健康体检者为健康组。应用功能性胃肠病症状自评量表进行问卷调查,比较两组FD发病情况及临床表现。结果观察组的FD发病率为27.8%(39/140),显著高于健康组的7.0%(7/100),两组间比较,差异有统计学意义(P<0.05)。观察组FD患者中,餐后饱胀、上腹烧灼感、上腹痛、早饱、恶心、呕吐、厌食、嗳气、混合多种症状发生率分别为11.4%(12/140)、1.4%(2/140)、2.1%(3/140)、2.8%(4/140)、0.7%(1/140)、0.7%(1/140)、1.4%(2/140)、 1.4%(2/140)、5.7%(8/140)。结论 Hp感染者FD发生率较高,以餐后不适综合征较常见,餐后饱胀和混合症状为主要表现。因此,应注意FD发病与Hp感染的相关性,积极完善相关检查,并予以个体化综合治疗。
Objective To investigate the incidence and clinical characteristics of functional dyspepsia(FD)in patients infected with Helicobacter pylori(Hp).Methods A retrospective study was performed on 140 cases of Hp positive patients who were admitted from July 2016 to October 2017.A total of 100 healthy subjects without Hp infection during the same period were selected as the healthy group.The incidence and clinical manifestations of FD in the two groups were compared by using the functional gastrointestinal symptom self-rating scale.Results The incidence of FD in the observation group was 27.8%(39/140),significantly higher than that in the healthy group(7.0%).The difference between the two groups was statistically significant(P<0.05).Among the FD patients in the observation group,the incidence of postmeal fullness,upper abdominal burning,upper abdominal pain,early fullness,nausea,vomiting,anorexia,belching and mixed symptoms were 11.4%(12/140),1.4%(2/140),2.1%(3/140),2.8%(4/140),0.7%(1/140),0.7%(1/140),1.4%(2/140)and 5.7%(8/140),respectively.Conclusion The incidence of FD is higher in Hp infected population.Postprandial fullness and mixture of different symptoms are the main clinical manifestations.Therefore,we should pay attention to the correlation between FD and Hp infection,improve the relevant examination and give individualized comprehensive treatment.
