摘要
目的:探讨PDCA循环管理方法在降低门诊药房调剂差错中的应用效果。方法:按照PDCA循环管理法,调查我院2015年、2016年门诊药房药品接近失误(内差)、药品差错(外差),分析差错及失误原因,并拟定措施、检查和落实,最后对实施效果进行评价。结果:针对门诊药房调剂差错的原因,完善管理制度,并与绩效挂钩,落实奖惩;量化岗位绩效(技术含量、工作强度、工作量);加强新进人员包括实习生带教及专业知识培训;加强药品管理,对易出错的药品设置警示标识;同时通过检查反馈及时调整药品货架。实施后,与2015年相比,2016年调剂差错指标有所下降,总差错发生率0.20‰,下降幅度23.01%。结论:应用PDCA循环管理方法能明显降低门诊药房调剂差错,使调剂差错率降低到预定目标值,是防范调剂差错的有效工具。
Objective:To investigate the application effect of PDCA circulation method in lowering the dispening error of out-patient in our hospital.Methods:According to the PDCA circulation method,drug close to failure and the incidence of drug errors in 2015,2016 were investigated to find out the problems;the reasons were analyaed and countermeasures were made out,checked and implemented.Finally,the quality of drug close to failure and the incidence of drug errors were compared before and after the implementation of PDCA circulation method.Result:Due to the reasons of drug close to failure and the incidence of drug errors,we formulated related countermeasures,such as perfecting the system of administration,contacting the performance with the benefit,implementing rewards and punishments mechanism,quantizing position performance(technical content、job intensity、workload),enhancing new-coming people,including internship teaching and training of professional knowledge,strengthening the management of drug,warning market of easy mistake,at the same time,by checking the feedback timely adjust drug shelves.After the implementation of countermeasures,compared to 2015,the index of dispensing errors were declined in 2016,such as the total incidence of errors is 0.20‰,decreased by23.01%compared with the same period last year.Conclusion:PDCA circulation method can significantly decrease the incidence of errors,drope the incidence of drug errors to a preset value,be an effective measures what avoids dispensing errors.
