硫酸镁与高压氧联合治疗胎儿宫内发育迟缓效果及对孕妇外周血中一氧化氮水平和前列腺素水平的影响 被引量:6

Effect of magnesium sulfate combined with hyperbaric oxygen on intrauterine growth retardation and its effect on NO level and prostaglandin level in maternal peripheral blood

摘要 目的观察硫酸镁与高压氧联合治疗胎儿宫内发育迟缓(intrauterine growth retardation,IUGR)效果及对孕妇外周血中一氧化氮(nitric oxide,NO)和前列腺素水平的影响。方法选取2016年3月至2017年3月黄冈市黄州区妇幼保健院收治的IUGR孕妇86例为研究对象,按照随机数字表法将其分为对照组与研究组,各43例。两组均给予常规治疗,对照组给予硫酸镁治疗,研究组在对照组的基础上联合高压氧治疗,持续治疗7 d。观察两组胎儿生长情况,记录两组治疗前后脐动脉血流参数、血液流变学指标水平,并检测两组孕妇治疗前后外周血中NO、6酮前列腺素F_(1α)(6 keto prostaglandin F_(1α),6-keto-PGF_(1α))、血栓素B_2(thromboxane B_2,TXB_2)水平。结果治疗后,研究组宫高、腹围、体重增长[(1.92±0.48)cm,(3.08±0.56)cm,(1 069.47±89.53)g]情况显著优于对照组[(1.67±0.41)cm,(2.75±0.51)cm,(976.52±84.35)g](P<0.05);两组脐动脉血流收缩期峰值/舒张期峰值(systolic peak value/diastolic peak value,S/D)、搏动指数(pulsation index,PI)、阻力指数(resistance index,RI)等参数均较治疗前显著降低(P<0.05),且研究组(2.31±0.25,0.72±0.09,0.53±0.06)显著低于对照组(2.46±0.28,0.83±0.11,0.58±0.07)(P<0.05);两组全血粘度低切(low shear blood viscosity,LBV)水平较治疗前显著降低(P<0.05),且研究组[(5.53±0.56)mPa·s]显著低于对照组[(5.85±0.59)mPa·s](P<0.05);两组外周血NO、6-keto-PGF_(1α)水平均较治疗前显著增加、TXB_2水平显著降低(P<0.05),且研究组显著优于对照组(P<0.05)。结论硫酸镁与高压氧联合治疗IUGR效果显著,可改善孕妇外周血NO水平和前列腺素水平。 Objective To observe the effect of magnesium sulfate combined with hyperbaric oxygen on intrauterine growth retardation(IUGR) and the effects on nitric oxide(NO) and prostaglandin levels in peripheral blood of pregnant women. Methods 86 cases of IUGR pregnant women admitted to the Huangzhou District Maternal and Child Health Hospital of Huanggang City from March 2016 to March 2017 were selected as the study subjects. According to the random number table method, they were divided into the control group and the study group, 43 cases each.The two groups were given conventional treatment, the control group was treated with magnesium sulfate, and the study group was treated with hyperbaric oxygen on the basis of the control group for 7 days. The fetal growth of the two groups was observed. The blood flow parameters and blood rheology indexes of the umbilical artery before and after treatment were recorded. The NO and 6-keto prostaglandin F1α(6-keto-PGF1α), thromboxane B2(TXB2) levels in the peripheral blood of the two groups before and after treatment were detected. Results After treatment, uterine height, abdominal circumference, weight gain [(1.92±0.48)cm,(3.08±0.56)cm,(1 069.47±89.53)g] were significantly better than those in the control group [(1.67±0.41)cm,(2.75±0.51)cm,(976.52±84.35)g](P<0.05). After treatment, umbilical artery peak systolic/diastolic peak(S/D), pulsatility index(PI) and resistance index(RI) and other parameters were significantly lower than those before treatment(P<0.05), and those in the study group(2.31± 0.25,0.72± 0.09,0.53± 0.06)were significantly lower than those in the control group(2.46± 0.28,0.83± 0.11,0.58± 0.07)(P<0.05).
