摘要
间歇期痛风病机为脾肾亏虚、痰湿瘀结,治疗以健脾补肾、祛湿泄浊、化痰瘀通络为主。目前研究显示中药复方能有效治疗间歇期痛风,部分疗效甚至优于西药,且部分研究显示中药治疗急性痛风复发率低于西药,提示中药复方在间歇期痛风治疗中具有独特的优势。但目前间歇期痛风的中药复方研究亦存在一些问题,如各研究的治疗目标是将血尿酸降至正常(<460 mol·L^(-1)),但间歇期痛风患者的治疗目标是将血尿酸控制在360 mol·L^(-1)以下,中药复方能否将血尿酸水平控制在此目标值暂无临床数据;另研究的周期尚短,停药后尿酸升高及急性痛风复发情况亦无追踪随访,故应充分利用现代化医疗研究环境,结合前人的研究成果,开展更多更全面更深入的研究。
The pathogenesis of intermittent gout is deficiency of spleen and kidney,phlegm-dampness and blood stasis.The main treatment is 'invigorating spleen and nourishing kidney,dispelling dampness and removing turbidity,resolving phlegm and blood stasis and dredging collaterals'.Current studies show that Chinese medicinal compound can effectively treat intermittent gout,some of which are even better than Western medicine,and some studies show that the recurrence rate of Chinese medicinals in treating acute gout is lower than Western medicine,which all suggest that Chinese medicinal compound prescription has unique advantages in the treatment of intermittent gout.However,there are still some problems in the study of Chinese medicinal compound prescriptions for intermittent gout.For example,the aim of each study is to reduce serum uric acid to normal(less than 460 mol·L-1),but the goal of treatment for patients with intermittent gout is to control serum uric acid to less than 360 mol·L-1.Whether TCM compound prescriptions can control serum uric acid level at this target value has no clinical data for the time being.In addition,the research cycle is still short,and there is no follow-up for the rebound of uric acid and the recurrence of acute gout after withdrawal.Therefore,we should make full use of the modern medical research environment and combine the previous research results to carry out more comprehensive and in-depth research.
