摘要
目的:观察经鼻内镜鼻中隔局部矫正术在鼻窦鼻中隔联合手术中的疗效。方法:对165例行经鼻内镜鼻中隔矫正+鼻窦手术的患者按照不同的鼻中隔矫正术式分为鼻中隔局部矫正术组(76例)和三线减张鼻中隔矫正术组(89例),按照指南规范治疗,所有患者进行术后24 h患者舒适度评价(VAS)、术后24周舒适度评价(VAS)和鼻内镜检查Lund-Kennedy评分。应用SPSS 20. 0软件进行统计分析。结果:局部矫正组与三线减张组术后24 h VAS评分有显著差异(P<0. 01),术后24周VAS评分及鼻内镜检查Lund-Kennedy评分无显著性差异(P值分别为0. 59、0. 08)。另外,两组患者24周VAS评分大于5的比率,分别为3. 95%和5. 62%;两组患者Lund-Kennedy评分大于5的比率分别为2. 63%和3. 37%。结论:部分类型患者行鼻中隔局部矫正术+鼻窦手术疗效确切,且明显减轻患者痛苦,更符合鼻内镜微创外科手术理念,鼻中隔局部矫正术是三线减张鼻中隔矫正术的有益补充。
Objective:To observe the curative effect of endoscopic surgery for chronic rhinosinusitis and deviation of nasal septum with limited septoplasty.Methods:165 patients underwent endoscopic sinus surgery and septoplasty were enrolled this study.Of these patients,76 underwent limited septoplas-ty,89 underwent septoplasty with three tension lines resection.All patients were evaluated by visual analogue scale(VAS)24 hours and 24 weeks after surgery.Then,all patients were detected by using nasal endoscope and evaluated with Lund-Kennedy score 24 weeks after surgery.SPSS 20.0 software was used for statistical analysis.Results:There was a significant difference in the VAS scores be-tween limited septoplasty group and the group with three tension lines resection 24 hours after opera-tion(P<0.01).There was no significant difference in VAS scores and Lund-Kennedy scores 24 weeks after operation(P=0.59,0.08 respectively)between the two groups.In addition,the ratio of VAS>5 for 24 weeks was 3.95%and 5.62%,and the Lund-Kennedy score was 5 and 2.63%,re-spectively in the two groups.Conclusion:As an effective treatment,endoscopic limited septoplasty with sinus surgery can relieve suffering after surgery.Limited septoplasty is complementary for septo-plasty with three tension lines resection.
