摘要
目的观察腹腔镜与胆道镜联合治疗胆囊畸形的临床疗效。方法回顾性分析我院普通外科2012年1月至2013年12月治疗的41例胆囊畸形患者的临床资料,所有患者均采用腹腔镜与胆道镜联合治疗,观察手术时间、术后排气时间、并发症发生情况及手术成功率;随访期间,观察术后临床症状改善情况,并检测胆囊收缩功能。结果手术成功率为100.00%,术后3例发生胆漏,发生率为7.32%;平均手术时间(107.3±19.7)min;平均排气时间(26.6±3.2)h;平均拔管时间(4.9±1.1)d;平均住院时间(5.4±1.6)d;38例患者随访至2015年9月,平均随访时间(23.7±4.1)个月,无胆囊切除术后腹胀、腹痛、腹泻、消化不良等并发症,术前症状消失,术后胆囊收缩功能为(52.7±10.7)%,较术前的(38.6±8.2)%明显提高,差异有统计学意义(P<0.05)。结论严格筛选手术适应证,采用腹腔镜与胆道镜联合治疗胆囊畸形不仅可保留胆囊,还可提高胆囊功能,改善临床症状,是治疗胆囊畸形的有效方法。
Objective To study the clinical effect of laparoscopic combined with choledochoscope in the treatment of gallbladder abnormalities. Methods The clinical data of 41 patients with gallbladder abnormalities in Department of General Surgery from January 2012 to December 2013 were retrospectively analyzed, which were all treated by laparoscopy combined with choledochoscopy. The operation duration, anal exhaust time, complications, and success rate of surgery were observed. During the follow-up, the improvement of postoperative clinical symptoms and gallbladder contractive function were detected. Results The success rate of surgery was 100.00%, and the incidence of postoperative bile leakage was 7.32%. The average operation duration, anal exhaust time, extubation time, length of hospital stay were(107.3 ± 19.7) min,(26.6 ± 3.2) h,(4.9 ± 1.1) d,(5.4 ± 1.6) d, respectively. Thirty-eight patients were followed up to September 2015, with the average follow-up of(23.7±4.1) months. No complications such as abdominal distension, abdominal pain, diarrhea, indigestion were observed after cholecystectomy, and preoperative symptoms disappeared. The postoperative gallbladder contractive function was(52.7±10.7)%, which was significantly increased compared with the preoperative(38.6±8.2)%, and the difference was statistically significant(P<0.05). Conclusion Strict screening of operative indication, applying laparoscopy combined with choledochoscopy for treating gallbladder abnormalities can not only keep the gallbladder, but also improve the gallbladder function, improve clinical symptoms, which is one of the effective methods to treat gallbladder abnormalities.
