摘要
目的探讨腹腔镜-胆道镜联合微创保胆治疗胆囊结石、息肉的临床效果.方法选取大理州人民医院收治的90例胆囊结石、息肉患者为研究对象,按照所用手术方式将患者分为观察组和对照组,2组患者分别行腹腔镜-胆道镜联合微创保胆术和腹腔镜胆囊切除术治疗,比较2组患者的一般手术情况、并发症、生活质量和满意度情况.结果观察组患者的术中出血量显著低于对照组(P<0.05),而手术时间、住院时间和术后肛门排气时间均与对照组差异无统计学意义(P>0.05);观察组患者的生理功能、主观症状、心理情绪、社会功能及生活质量总分均显著高于对照组(P<0.05);观察组患者并发症发生率显著低于对照组(P<0.05);观察组患者对手术的总满意度显著高于对照组(P<0.05).结论腹腔镜-胆道镜联合微创保胆治疗胆囊结石、息肉术中出血少,术后并发症发生率低,患者生活质量和满意度高,值得在临床推广使用.
Objective To investigate the clinical efficacy of laparoscopy-choledochoscopy combined with mi-croinvasive gallbladder protection therapy in treating cholecystolithiasis and gallbladder polyps. Methods A total of90 cholecystolithiasis and gallbladder polyps cases admitted to people＇s hospital of Dali were recruited in this study,and were further categorized into observation group and control group based on surgical method. Laparoscopy-chole-dochoscopy combining with microinvasive gallbladder protection therapy was provided for observation group, andlaparoscopic cholecystectomy was provided for control group, then general operation condition, complication,living quality and satisfaction degree between two groups of patients were compared. Results Intraoperative bleedingin observation group was significantly lower than control group （ P〈0.05）, while no significant difference wasfound in operation duration, hospital stay and postoperative anal aerofluxus between two groups （ P〉0.05）;physiological function, subjective symptom, psychological status, social function and total score of living qualityin observation group were significantly higher than those in control group （P 〈0.05） ;the incident rate of complica-tions in observation group was significantly lower than control group （P 〈0.05）;the overall satisfaction degree in observation group was significantly higher than control group （ P〈0.05） .Conclusions In the treatment of chole-cystolithiasis and gallbladder polyps,laparoscopy-choledochoscopy combining with microinvasive gallbladder pro-tection therapy can reduce intraoperative bleeding and incident rate of postoperative complications,improve patientliving quality and satisfaction degree,so it is worthy of promoting in clinical practice.
