摘要
为了解病毒性心肌炎 (VMC)患儿抗心磷脂抗体 (ACA)变化与心脏损害的关系 ,以及肾上腺皮质激素对ACA免疫球蛋白 G(Ig G)的影响 ,进行了动物实验和临床研究。将 16 0只小鼠分为四组 ,A组不感染病毒 ,B组感染柯萨基病毒 B3(Cox B3) ,C组感染 Cox B3病毒早期加强的松 ,D组感染 Cox B3病毒后期加强的松 ,测定其血清中ACA Ig G。将 92例 VMC患儿分为常规治疗组和激素治疗组 ,两组治疗前均测定血清 ACA Ig G、心电图、肌酸激酶同工酶 (CK- MB)和心脏功能 [搏血指数 (SI)、心脏指数 (CI)、射血分数 (EF)、短轴缩短率 (FS) ],并于治疗后 1个月复查血清 ACA Ig G。结果显示 ,Cox B3感染小鼠 ACA Ig G阳性率均显著高于未感染小鼠 ,激素治疗后显著降低。VCM患儿 ACA Ig G阳性率显著升高 ,治疗 1个月后阳性率显著下降 ,以激素治疗组下降明显。 ACA Ig G阳性VMC患儿的心电图 ST- T异常率、CK- MB升高率、心功能降低率均显著高于 ACA Ig G阴性患儿。提示 VMC小鼠及患儿 ACA Ig G显著升高 ,ACA Ig G阳性者病情显著重于阴性者 ,强的松能显著降低 ACA Ig
To realize the change of antiphospholipin antiboby immunoglobulin G(ACA IgG)and the relationship between ACA IgG and heart injured,and to realize the influence of corticosteroid to ACA IgG in the treatment of viral myocarditis(VMC).160 mice were divided into 4 groups(Group A:normal control.Group B:CoxB 3 infected.Group C:CoxB 3 infected+prednisone early used.Group D:CoxB 3 infected+prednisone late used).ACA IgG in blood was measured.92 cases with VMC were divided into 2 groups(62 cases were treated with general treatment method,30 cases were treated with prednisone).ACA IgG were examined before treatment and one month later.The EKG,heart function,CK MB were examined every case before treatment.Results showed that the ACA IgG positive rate was significantly higher in mice infected by CoxB 3 than normal mice.The ACA IgG rate was significantly decreaced by prednisone treatment,the ACA IgG positive rate of the children with VMC were significantly higher than that of normal children,and were siginificantly decreased after treatment.ACA IgG positive rate were significantly lower in patients treated by prednisone than by general treatment.ST T abnormal rate,CK MB increasing rate and heart function decreasing rate was significantly higher in VMC children whose ACA IgG was positive than in children whose ACA IgG was negative.These suggest positive rate of ACA IgG of VMC children increase.VMC children are more severe in VMC children whose ACA IgG is positive than in VMC patients whose ACA IgG is negative.Prednisone treatment could decrease the positive rate of ACA IgG in patients with VMC.
