摘要
目的 :探讨乳腺癌患者负性生活事件与细胞免疫的相关因素。方法 :选择 1998～ 2 0 0 0年住本科治疗并经病理学或细胞学确诊的乳腺癌患者 115例 ,乳腺良性疾病患者 115例 ,按同性别、同职业、文化程度相当、生活习惯相近、年龄相仿的原则分组比较。根据杨德森编制的生活事件量表 (lifeeventscaleLES) ,对家庭生活问题、工作学习问题、社交及其他问题进行对比调查 ,同时检测细胞免疫功能。结果 :①乳腺癌组负性生活事件的总分值显著高于对照组 (P <0 0 0 1) ,而CD3 (T细胞总数 )明显降低(P <0 0 0 1)。②乳腺癌腋淋巴结转移组与非转移组T细胞亚群无变化。③乳腺癌组随着负性生活事件分值的升高腋淋巴结转移数增多。④乳腺癌的不同组织学类型其细胞免疫指标不同 (P <0 0 5 )。⑤不同年龄的乳腺癌其细胞免疫指标有差异。⑥乳腺癌不同职业、不同文化程度与负性生活事件、细胞免疫各组无差异。⑦良性组随着年龄的增大负性生活事件分值增多 ,细胞免疫指标下降。结论 :乳腺癌的发生与负性生活事件关系密切 ;负性生活事件与机体的细胞免疫相关 ,并首次发现负性事件对乳腺癌的淋巴结转移有影响 ;提出在研究乳腺癌的病因学方面 ,应重视负性情绪与人体内分泌、免疫功能以及肿瘤转移的关系 ,为乳腺癌防治提出新思?
Objective To evaluate the relationship between negative life events with cellular immunity, lymphatic metastasis and histology in breast cancer.Methods 115 cases of patients with breast cancer and 115 cases with benign breast diseases from 1998 to 2000 were enrolled in this study.The patients were divided and compared according to sex,age,occupation,education, and living custom.The comparison was carried out in accordance with Life Event score (LES) established by Yang Eesheng.The cellular immunity was also tested.Results The total scores of negative life events in breast cancer patients were higher significantly than those of control group ( P <0 001);CD 3 (total T cells) in breast cancer was reduced markedly.The difference between two groups was statistically significant ( P <0 001).Nevertheless,CD 4(helper T cell),CD 8 (suppressor T cell),the ratio of CD 4 to CD 8 and NK cells in the two groups had no obvious difference ( P <0 05);More lymphatic metastasis were observed as the negative life events increased;There was no difference in cellular immunity index at different histological types of breast cancer.Conclusions The occurrence of breast cancer was relative with negative life events; negative life events with the decrease of total T lymphocytes but not relative with the percentage of other cells;negative life events increased the chance of lymphatic metastasis in breast cancer;the immunity index at different breast cancers existed difference ( P <0 05).
