摘要
目的 探讨超声测量下腔静脉塌陷指数对肺栓塞筛查的价值。方法 利用二维超声、彩色多普勒超声对 3 1例急性、慢性肺栓塞患者和 3 1例常规体检者进行下腔静脉塌陷指数测量。选择空腹检查条件 ,患者取仰卧位 ,充分显示距下腔静脉入右心房 2cm以内纵断面的管腔结构 ,以彩色多普勒超声证实此段血流通畅 ,而后测量吸气、呼气时下腔静脉的内径。结果 3 1例急性、慢性肺栓塞患者下腔静脉塌陷指数下降占 19例 (5 9.4% )。大块肺栓塞组、非大块 /次大块肺栓塞组和正常对照组的呼气末下腔静脉内径、吸气末下腔静脉内径和下腔静脉塌陷指数分别为 (1.78± 0 .70 )cm ,(1.49± 0 .61)cm ,0 .14 8± 0 .0 974;(1.0 6± 0 .3 8)cm ,(0 .5 0± 0 .2 1)cm ,0 .5 17± 0 .0 75 ;(2 .3 7± 0 .3 4)cm ,(0 .46± 0 .2 2 )cm ,0 .80 5± 0 .115。结论 无创伤性超声技术对急性、慢性肺栓塞筛查具有较高特异性 ,单独根据下腔静脉塌陷指数判断肺栓塞可能会出现较高的假阴性。
Objective To investigate the ultrasound technic value of diagnosis in the pulmonary embolism by the collapsible index of inferior vena cava. Methods The collapsible index of inferior vena cava were measured in 31 acute and chronic patients with pulmonary embolism and 31 routine health people applying gray scale imaging,color and spectral Doppler.We asked the patients to be on an empty stomach with supine position, manifested the structure of inferior vena cava before it entered the right atrium within 2cm in vertical section,proved the extent of inferior vena cava to be unobstructed by color Doppler,and measured the bore of inferior vena cava at the telophase of inspiration and expiration. Results The descent of collapsible index of inferior vena cava in 19 acute and chronic patients (19/31) with pulmonary embolism was found.The expiratory and inspiratory diameters of inferior Vena and the collapsible index of inferior Vena Cava in group of massed pulmonary embolism,non massed pulmonary embolism and control group were (1.78±0.70)cm,(1.49±0.61)cm, 0.148±0.0974 ;(1.06±0.38)cm,(0.50±0.21)cm,0.517±0.075 and (2.37±0.34)cm,(0.46±0.22)cm,0.805±0.115 respectively. Conclusion The specificity of noninvasive ultrasonography in 31 acute and chronic patients with pulmonary embolism was higher than the sensibility.There will be high false negative when the pulmonary embolism was diagnosed by the collapsible index of inferior vena cava alone.
