心脏常温机械灌注装置的研制

摘要 目的研制一种心脏常温机械灌注装置,以实现移植心脏的离体保存。方法设备利用蠕动泵和膜式氧合器模拟人工心肺,通过给离体心脏顺行灌注常温含血灌注液,实现移植心脏体外节律搏动及长时间保存。同时通过灌注液乳酸水平判断心脏灌注状态,实时监测心脏灌注流量及压力。结果本设备的重要参数可基本满足需求,且已经在中华小型猪心脏(n=3)上进行离体灌注实验。结果显示心脏离体后搏动良好,初步验证了其可行性。结论常温机械灌注为心脏移植器官保存提供了新思路,且有望延长心脏离体保存时间,扩大潜在供体器官数量。 Objective To introduce a heart normothermic machine perfusion device to achieve in vitro preservation of transplanted hearts.Methods The peristaltic pumps and membrane oxygenators were utilized to simulate an artificial heart and lung.The solution of infusing normothermic blood perfusate to the transplanted heart was used to achieve rhythmic beating and long-term preservation of the transplanted heart in vitro.At the same time,The cardiac perfusion status was determined by the lactate level of the perfusate,and the cardiac perfusion flow and pressure were monitored in real-time.Results The device parameters met the necessary requirements,and it had been used for in vitro perfusion experiments in Chinese miniature porcine hearts(n=3).The results showed that the heart was well paced in vitro,which preliminary validated its feasibility.Conclusion Normothermic machine perfusion provides a novel strategy for heart preservation and is expected to extend the preservation time of the heart in vitro and expand the number of potential donor organs.
