人体成分与绝经后骨质疏松患者心电图的相关性分析

Correlation analysis between body composition and electrocardiogram in postmenopausal osteoporosis patients

摘要 目的探究绝经后骨质疏松人群中人体成分与心电图异常以及对营养干预的指导意义。方法回顾性分析2019年9月—2022年9月本院收治的确诊为骨质疏松已绝经女性患者181例的临床资料,分为心电图异常组及心电图正常组,比较两组相关临床指标差异。采用人体成分分析仪(inbody770)分析两组身高、体重、体质指数、腹围、臀围、上臂肌围、身体总水量、细胞内水分、细胞外水分、矿物质、蛋白质、骨矿物质、去脂体重、肌肉量、四肢骨骼肌质量、体脂肪、体脂百分比、InBody评分、内脏脂肪面积、身体细胞量、相位角的差异。采用二元logistic回归分析,探讨有无心电图异常的独立危险因素。结果两组患者临床指标比较高密度脂蛋白胆固醇差异有统计学意义(P<0.05),人体分析比较在心电图异常组的体重、BMI、腹围、臀围、上臂肌围、矿物质、骨矿物质、体脂肪、体脂百分比、内脏脂肪面积较心电图正常组偏高,InBody评分则偏低。内脏脂肪面积是有无心电图异常的独立危险因素。心电图异常组,内脏脂肪面积与高密度脂蛋白胆固醇呈负相关(r=-0.318,P=0.01),且高内脏脂肪面积组发生ST-T波改变、QTc间期延长、概率更高。结论绝经后骨质疏松患者内脏脂肪面积可预测心电图异常的风险,对相关人群可进行针对性营养干预。 Objective To explore the correlation between body composition and electrocardiogram(ECG)abnormalities in the postmenopausal osteoporosis population and the guiding significance for nutritional intervention.Methods Retrospective analysis was conducted on the clinical data of 181 postmenopausal women diagnosed with osteoporosis admitted to our hospital from September 2019 to September 2022.They were divided into two groups:the abnormal ECG group and the normal ECG group.Clinical index differences between the two groups were compared.A body composition analyzer(InBody 770)was used to analyze differences in height,weight,body mass index(BMI),waist circumference,hip circumference,upper arm circumference,total body water,intracellular water,extracellular water,minerals,protein,bone minerals,fat-free mass,muscle mass,appendicular skeletal muscle mass,body fat,body fat percentage,InBody score,visceral fat area,body cell mass,and phase angle.Binary logistic regression analysis was used to explore the independent risk factors for ECG abnormalities.Results There were statistically significant differences in high-density lipoprotein cholesterol between the two groups(P<0.05).In the body composition analysis,the abnormal ECG group had higher body weight,BMI,waist circumference,hip circumference,upper arm circumference,minerals,bone minerals,body fat,body fat percentage,and visceral fat area compared to the normal ECG group,while the InBody score was lower.Visceral fat area was identified as an independent risk factor for ECG abnormalities.In the abnormal ECG group,there was a negative correlation between visceral fat area and high-density lipoprotein cholesterol(r=-0.318,P=0.01),and a higher probability of ST-T wave changes and prolonged QTc interval in the high visceral fat area group.Conclusions Visceral fat area can predict the risk of ECG abnormalities in postmenopausal osteoporosis patients,and targeted nutritional interventions can be conducted for these populations.
