摘要
目的分析中晚期非小细胞肺癌(non-small cell lung cancer,NSCLC)患者血清微小RNA-244(microRNA-244,miR-244)表达及其与患者临床病理特征的关系。方法选取2022年1月—2023年6月徐州医科大学附属医院呼吸与危重症医学科收治的80例NSCLC患者作为NSCLC组,同时选取60例同时期的良性病变患者为对照组。测定2组miR-224表达水平,分析其与中晚期NSCLC患者临床病理特征的关系。结果对照组miR-224表达水平低于NSCLC组,差异有统计学意义(P<0.05)。治疗后,Ⅲ~Ⅳ期NSCLC组miR-224表达水平降低,差异有统计学意义(P<0.05);Ⅲ~Ⅳ期NSCLC组在不同病理分期、病理分级、淋巴结转移中,miR-224表达水平比较差异有统计学意义(P<0.05)。miR-224表达水平的曲线下面积(area under the curve,AUC)为0.845,差异有统计学意义(P<0.05),敏感性、特异性、最佳截断值为85.00%、86.70%、2.83;以2.83为参考值,miR-224表达水平检出阳性76例、阴性64例,其与病理结果具有较强一致性(Kappa值=0.711,P<0.001)。结论中晚期NSCLC患者miR-224表达水平与病理分期、病理分级、淋巴结转移具有相关性,且其在诊断NSCLC中有较为准确的价值。
Objective To analyze the expression of serum microRNA-244(miRNA-244)in patients with advanced nonsmall cell lung cancer(NSCLC)and its relationship with the patients'clinicopathological characteristics.Methods A total of 80 patients with NSCLC admitted to department of respiratory and critical care medicine,the Affiliated Hospital of Xuzhou Medical University from January 2022 to June 2023 were selected as the NSCLC group,and 60 patients with benign lesions in the same period were selected as the control group.The expression levels of miR-224 were measured in both groups and analyzed for their relationship with the clinicopathological characteristics of patients suffering from middle and late stage NSCLC.Results The expression level of miR-224 in the control group was lower than that in the NSCLC group,and the difference was statistically significant(P<0.05).After treatment,the expression level of miR-224 in stage Ⅲ-Ⅳ NSCLC group decreased significantly(P<0.05).In the group of stage Ⅲ-Ⅳ NSCLC,there was a statistically significant difference in the expression levels of miR-224 among different pathological stages,pathological grades,and lymph node metastasis(P<0.05).The area under the curve(AUC)of miR-224 expression level was 0.845,and the difference was statistically significant(P<0.05).The sensitivity,specificity,and optimal cutoff values were 85.00%,86.70%,and 2.83,respectively.Using 2.83 as a reference value,miR-224 expression levels were detected in 76 positive cases and 64 negative cases,which showed strong consistency with the pathological results(Kappa value=0.711,P<0.001).Conclusion The expression level of miR-224 in patients suffering from middle and late stage of NSCLC is correlated with pathological stage,pathological grade and lymph node metastasis,and it has an accurate value in the diagnosis of NSCLC.
